Job overview
An exciting opportunity has become available for a Band 6 Nurse to join the South Lakes Older Adult Community Mental Health Team for 22.5 hours per week. The team are looking for a dynamic colleague who shares our passion for ensuring that individuals enjoy great care and health regardless of age and diagnosis.
We are looking for a forward thinking, caring individual who has excellent leadership qualities and experience of working with mental health difficulties relating to the ageing process and people who have a dementia diagnosis. Experience in a care home setting is desirable as well as robust assessment skills.
Main duties of the job
• To work within the OACMHT in collaboration with service users, carers, Occupational Therapy, Psychology, and Primary Care, to be responsible for the assessment and management of service users who have substantial and complex needs.
• Provide specialist clinical advice and education to service users/carers, staff, statutory agencies, 3rd sector partner agencies and the public.
• In collaboration with the service user and carer, develop individualised focused care plans, ensuring regular reviews to monitor progress and adapt care plans as necessary.
• To consider carer needs and ensure carer and family are involved in care planning where appropriate
• To deliver evidenced based Clinical and Psychologically based Interventions.
• The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence/ occupation.
• Coordinate treatment planning & interventions, reviews and discharge planning utilising dialog + care planning.

Qualifications
Essential criteria
- First Level Registered Nurse RNMH, RMN,
- Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post
- Proven track record of post registration clinical experience relevant to the post.
Knowledge
Essential criteria
- Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.
Desirable criteria
- Understanding of difficult to manage/challenging behaviours.
- Understanding delirium and dementia.
Experience
Essential criteria
- Experience of Leadership, Experience of mentoring teams and achieving successful outcomes.
- Experience of working with people with a range of MH conditions.
- Experience of caring for people with Dementia.
