Diabetes Specialist Nurse

West London NHS Trust  provides a full range of mental health, community  and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.

We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

We have an exciting opportunity for a Band 7 Diabetes Specialist Nurse to join our award-winning proactive Hounslow Community Diabetes Service.

We are seeking an experienced Diabetes Specialist Nurse who has the passion and skills for improving the care of people with Type 2 Diabetes (and Type 1 Diabetes in special circumstances). The role would suit someone who is highly motivated with a positive approach to high quality care delivery with excellent communication and leadership skills to work in partnership with our community and primary care colleagues.

Experience of working with virtual technology to support a flexible service in complex Diabetes clinics and group settings would be welcome. The role includes provision of respectful evidence-based care in nurse-led step up and step-down clinics service; and jointly care for housebound patients and patients with special needs to enable supported self-management skills for patients and their carers in partnership with the community team.

The service supports a consultant-led multidisciplinary team clinic.

Diabetes Specialist Dietitians work closely with the Diabetes Specialist Nurses and oversee the delivery of our X-PERT Structured Education Programmes for patients with Type 2 Diabetes.

The post is based at the Heart of Hounslow Centre for Health.

Prif ddyletswyddau'r swydd

• The post holder will work effectively as a member of the multidisciplinary team which includes patients, Specialist Diabetes Nurses, Diabetes Specialist Dieticians, Diabetes Consultant, Primary Care colleagues and community teams, to improve holistic health outcomes for patients with Diabetes.

• Provide specialist care, advice and support through direct booking Primary Care hubs (PCN) and jointly support the development of integrated working.

• Provide joint practice-based clinic sessions to optimise care planning and prescribing decisions for complex patients in GP practices.

• Triage and audit referrals to the service and provide relevant service and evidence-based feedback to referrers to provide timely care to patients.
