An exciting development opportunity has arisen for an experienced, motivated and committed nurse with a proven track record in Cardiology Nursing to join the Community Heart Failure Team with Lancashire and South Cumbria NHS Foundation Trust.

The Heart Failure service is for patients aged 18 years and above and is offered to patients who have a confirmed diagnosis of heart failure. Care will be delivered within a variety of locations across Greater Preston, Chorley and South Ribble. The service is predominantly provided within a community clinical environment, or home visits for those patients that have reduced mobility, require complex care and are housebound.

As part of the team you will be required to work autonomously prioritising your work load. You will possess significant cardiology experience and will provide support to all team members working together to optimise and titrate medications, provide symptom management and deliver education to aid improved quality of life following heart complications.

You will have excellent communication skills, be a good team player and have the ability to work under pressure alongside other members of the team, to provide a proactive and responsive service. You will have an in depth knowledge of cardiology care and will be supported to develop your skills and experience in Heart Failure. 
The post holder will be expected to work with the existing team and be able to commute to relevant community locations to deliver nursing care. The post holder must have proven experience within cardiology and nursing.

A flexible and dynamic approach to change is also required. In addition access to a car for work purposes is required to carry out home visits within the locality.
