This post falls is based in the South West London ICB,  Merton and Wandsworth Continuing Care Team in which staff support each other in service delivery across all five ICBs where the service is commissioned. This will require staff to work across the ICBs sites to meet the needs of patients and the service. The post holder will be responsible for the assessment of individuals in any setting (i.e. home, acute, care home) to determine eligibility to receive Fully Funded NHS Continuing Healthcare [CHC] or Funded Nursing Care [FNC].

The post holder will act as nurse assessor/co-ordinator of a Multidisciplinary Team to establish eligibility and working with the individual and their family identify a suitable care package and to ensure the review process of all Funded Nursing Care is undertaken in accordance with the National Framework Guidance for Continuing Healthcare (revised 2012).

·      The post holder will manage referrals from a range of professionals, patients and their carers determining eligibility for FNC or NHS CHC.

·       Using national assessment tools and adhering to the lead times, to co-ordinate, participate in and support the assessment process by the Multidisciplinary Team.

·    To determine the eligibility, as set out in the National Framework Guidance for Continuing Healthcare (revised 2012) and be responsible for explaining clearly to patients, families, health and social care colleagues and care providers so that the process is transparent and understood.

·  To undertake assessments, maintain appropriate written records and communicate information as required with all relevant parties whilst maintaining appropriate confidentiality of the patient’s records.

·    To restore and maintain independence, promote individual choice and self-determination, enabling clients to live in the community and support their preferences whenever feasible.

NHS South West London Integrated Care Board (ICB), as part of South West London Integrated Care System (ICS), is a partnership of organisations that come together to plan and deliver joined up health and care services to improve the lives of people in our six boroughs: Croydon, Merton, Kingston, Richmond, Sutton and Wandsworth.

Each ICS consists of two statutory elements:

  • enhance productivity and value for money and;
  • help the NHS support broader social and economic development.

·       To work across health and social care boundaries to obtain further specialist evidence and ensure a coordinated approach to care to educate, inform and involve clients, carers and Nurse Assessor Band 6 professionals on the National Framework Guidance for Continuing Healthcare (revised 2012) and matters of local service provision.

·     To be actively involved in complaint investigations and contribute to responses to appeals and in monitoring the quality care standards in local nursing homes. They will ensure the Head of

·       Continuing Care and the Commissioning Team are kept fully aware of any cause for concern.

·  To provide reports and advice when required in relation to Protection of Vulnerable Adults Procedures.

· To maintain registration, skills and knowledge including an awareness of policies, procedures and legislation laid out by all the relevant professional bodies.

·    To work within the clinical and policies guidelines and use initiatives and relevant practice for the Continuing Healthcare Service, ICB and CSU.

·     To participate in research, audit, clinical and peer supervision on a regular basis to develop appropriate personal objectives and a lifelong learning plan.

·    To develop and maintain multi-professional and agency networks representing the South West London ICB and to improve access for technical expertise and services to support better patient services.

·   To provide awareness of the professional role and education of referrers e.g. GPs, consultants, care managers, discharge coordinators, therapists, other professions and the voluntary sector.

·    To prioritise referrals and manage caseloads allocating resources and reviewing progress and productivity.

·   To call, attend or lead team, CHC and Multidisciplinary meetings, including case conferences and discharge meetings to ensure continuity of care across agencies.

·    To promote effective working arrangements with care homeowners, social services, community nursing and acute sector staff.

·      To undertake and comply with all mandatory training.

·    To undertake any other duties commensurate with the grade and responsibilities of the post.

Education /Qualifications

  • Educated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area.
  • Registered Health Professional - Current & relevant NMC registration
  • Degree or equivalent level of experience in health or social care

  • Academic/professional qualification at higher degree level or equivalent

Knowledge and Experience

  • Relevant nursing experience gained in a complex, multi-disciplinary environment
  • Experience of reporting clinical and non- clinical accidents and incidences promptly and when requested to co-operate with any investigation undertaken
  • Excellent knowledge and practical experience of the Continuing Healthcare Framework
  • Relevant nursing experience gained in a complex, multi-disciplinary environment

Skills and Abilities
