Mental Health Practitioner (RMN/OT)

Our Vision is to provide the best mental health, learning disability, autism, community, and physical health services for the populations we serve.
As an integrated Trust, Lancashire and South Cumbria NHS Foundation Trust (LSCFT) delivers a comprehensive range of services, including:
- Primary and secondary mental health care for children and adults, including specialist inpatient child and adolescent mental health provision, perinatal mental health, and forensic services with low and medium secure care.
- Specialist mental health inpatient care for individuals with learning disabilities, alongside specialist community support for children and adults with learning disabilities and autism, including intensive support.
- Extensive community physical health and well-being services for children and adults, covering prevention, treatment, rehabilitation, and long-term condition management.
The Trust was first established in 2002 and employs approximately 7,500 staff who provide care from more than 400 sites. The organisation offers opportunities for medics, mental health, general nurses, children’s nurses, allied health professionals, psychology, administration and clerical staff, apprentices and volunteers, as well as those specialising in learning disabilities and community physical health services.

Fylde and Wyre Community Mental Health Team has an exciting opportunity for a Band 6 Community Mental Health Practitioner to join the team. This post provides the opportunity for a forward thinking and enthusiastic individual to develop and implement safe, sound and supportive services that embrace the provision of holistic care. With a strong emphasis on multidisciplinary working and the principles laid out in the Recovery Model, you will need to be an innovative thinker and be able to form effective relationships.
You will have a working knowledge of assessments, care planning and the risk assessment and management of people under our community services, and the ability to explore issues relating to the impact of health and social needs on mental health and well-being.
Fylde and Wyre CMHT works over 2 bases due to the geographical size of the catchment area. You will have a main team base but there is the expectation of support across both sites as required.

• To undertake mental health assessments including Risk assessments within a multi-disciplinary assessment team.
• To be proficient in referring to other clinicians within the service or referring to other services/agencies where necessary.
• To provide a fast, responsive service to assess individual’s needs.
• Providing a communication structure between GP’s, IAPT and non IAPT services in LSCFT, non-statuary service, as a means of ensuring the highest possible standards of patient care.

- Registered Mental Health Nurse / Occupational Therapist.
- Degree/Diploma.
- On-going registration with the NMC/HCPC.
- Evidence of commitment to continuing professional development.
Knowledge

- Understanding of Health and Safety policies and procedures.
- Knowledge and practice of Mental Health Act 1983.
- Knowledge and practice of the Mental Capacity Act 2005.
- Demonstrate understanding of how research has influenced practice.
- Knowledge of common and complex mental health problems and medication used in relation to these.
Experience

- Able to work as part of a multidisciplinary team.
- Experience of assessing, planning and implementing and reviewing care needs.
- Experienced in acting as a mentor/assessor of student staff.
