West London NHS Trust  provides a full range of mental health, community  and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.

We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

Job overview

The Advanced Clinical Practitioner (ACP) will be embedded within Hammersmith and Fulham Central Primary Care Network (PCN), working collaboratively across five General Practices and the Central MINT (Community Adulth) service to deliver integrated, person-centred care addressing both mental and physical health needs.

The role combines autonomous advanced clinical practice with leadership, education, research, and service development, in line with the NHS Long Term Plan and the Multi-Professional ACP Framework.

The ACP will function as a senior mental health liaison within primary care, undertaking specialist assessments, formulating diagnoses, and delivering evidence-based interventions. The post holder will play a key role in service transformation by embedding mental health expertise within GP practices, strengthening continuity of care, and supporting the development, delivery, and governance of psychologically informed interventions provided by trainees and students.

Main duties of the job

  • To be responsibility for achieving defined targets and standards against the key performance and quality indicators.
  • Be fully conversant with the relevant indicators, able to analyse performance trends, identifying risk and advising the service manager.
  • Prepare accurate performance reports as required, formulate action plans to improve performance if necessary. Assist in the implementation of CIS providing clear leadership and taking action to ensure it is successful.
  • To ensure that all risk is managed in the relevant frameworks.  To develop, implement and maintain systems, processes and practice in risk management, including the management of Health and Safety.
  • To ensure that all aspects of Safeguarding are adhered to and that policies and procedures are followed at all times.
  • To be responsible that complaints are investigated fully, and are satisfactorily resolved ensuring a proactive approach is standard as part of the trust and local authority policies.

 This role will include working with people from varied cultural backgrounds and ages, using interpreters when necessary; a passion for creating and enabling equality of access to services for all is vital.

Qualifications

Essential criteria
  • Educated to degree/ diploma level with a relevant post- qualification or equivalent experience (Advance Clinical Practitioner Training) A qualification, or have undertaken a program of formal training in Clinical Supervision
  • Any other relevant leadership or relationship management qualification
  • Clinical/post graduate management qualification or equivalent level of experience and proven ability to perform at this level
Desirable criteria
  • A professional qualification in a core profession and registration with the associated professional body e.g. RMN, Occupational Therapist, Social Worker, Clinical Psychologist, Psychotherapist - Degree/diploma level supplemented by specialist qualification, training, experience, courses relevant to the CAMHS
  • Independent Non-Medical Prescriber Specialist Qualification in CBT, Family Therapy or other appropriate therapy

  • Demonstrable experience of working with adults with mental health problems in a variety of community settings Experience in primary care, community mental health, or urgent care settings
