We are seeking a motivated and compassionate Band 6 Early Intervention in Psychosis (EIP) Practitioner to join our dynamic multidisciplinary EIP team in Ealing.

This is an exciting opportunity to work with young people (aged 14 plus) and adults experiencing a first episode of psychosis, providing timely, evidence‑based, and recovery‑focused interventions.

You will play a key role in delivering high‑quality assessment, care coordination, and therapeutic support, working collaboratively with service users, families, and partner agencies to promote hope, resilience, and long‑term recovery.

Key Responsibilities

• Provide comprehensive biopsychosocial assessments for individuals experiencing first‑episode psychosis
• Act as a care coordinator for a defined caseload, delivering person‑centred, trauma‑informed care
• Develop and implement collaborative care plans, crisis plans, and risk management plans
• Deliver evidence‑based interventions, including CBT‑informed approaches, psychoeducation, and relapse prevention
• Work closely with families and carers, offering support and family‑focused interventions
• Participate in MDT meetings, contributing to clinical decision‑making and service development
• Liaise with internal and external partners including primary care, crisis services, acute wards, education providers, and voluntary sector organisations
• Support physical health monitoring and promote healthy lifestyle interventions
• Maintain accurate, timely clinical documentation in line with Trust policies
• Participate in audit, quality improvement, and service evaluation activities
