Surrey and Borders is looking for someone to work within a community team based within the Surrey Heath area working in partnership with statutory and voluntary sector to facilitate whole system approach to care delivery. The Post Holder will act as Care Coordinator/Lead Professional, where appropriate, to patients who require the skills of a Mental Health Nurse, and provide a holistic, clinical autonomous nursing service, to enable individuals to engage in the best standards of care available.

Working Pattern: 37.5 hours a week

Monday to Friday

9AM to 5 PM

  • Provide assessment, formulation, nurse diagnosis, and treatment options for individuals presenting with mental health problems, supporting Primary Care through regular liaison and meetings.

  • Deliver a high-quality community nursing service to older adults with mental health needs within a multi-professional framework, including lone working in line with local and Trust policies.

  • Prescribe and coordinate services to meet service user and carer needs in collaboration with the multidisciplinary team.

  • Attend MDT meetings, prepare reports, and participate in the team’s Duty Rota system.

  • Act as Key Worker/Care Coordinator or Lead Professional for named individuals under the Care Programme Approach (CPA), including completion of Single Assessments.

  • Assess and manage referrals through CMHT allocation meetings, determining suitability for specialist services.

  • Provide specialist advice and support to Primary Care services, including participation in integrated team working structures.

  • Administer prescribed medications, including intramuscular depot injections, and ensure safe delivery of oral medication in the community in accordance with national and Trust policies.

  • Support care and nursing homes in managing residents with mental health needs.

  • Act as a health advisor and educator across primary and secondary care settings, promoting positive physical and mental health approaches.

  • Discharge professional duties in accordance with the Mental Health Act 1983, Mental Capacity Act 2005, and the Nursing and Midwifery Council Code of Conduct, acting as a professional role model at all times.

  • Ensure safe storage, administration, and recording of medicines in line with national and Trust governance standards.

  • Maintain accurate, contemporaneous clinical records in accordance with the Caldicott Report principles and organisational expectations for a Band 6 clinician.

  • Take overall responsibility for the management of a defined caseload, ensuring care is evidence-based, regularly reviewed, and responsive to assessed need.

  • Provide clinical and management supervision to junior staff and students, including mentoring and competency development.

  • Develop and maintain a personal learning plan through supervision to ensure continued professional development and best practice standards.

  • Report incidents and complaints in accordance with Trust policy, utilise incident reporting systems appropriately, inform the Team Leader, and cooperate fully with investigations.

  • Build and maintain effective working relationships with statutory and voluntary services, particularly Social Services and Primary Care, contributing to integrated team working.

  • Ensure high standards of professional care delivery, safeguarding service quality and the reputation of the service.
