Surrey and Borders is looking for someone is passionate about developing and providing high quality, evidence based care and treatment to young people with moderate to severe emotional and mental health problems.
Working Pattern: Monday to Friday, 9AM to 5 PM
Work Location: Unither House, Curfew Bell Road, Chertsey, KT16 9FG
Job Purpose
- Report to the Service Manager and provide leadership in the development of the pathway and delivery of service.
- To provide timely assessment of children and young people who present with moderate to severe mental health needs, including deliberate self- harm assessments and provide a timely follow up appointment and signpost appropriately.
- To provide support, advice and education to the wider network including the Hub Team. To build knowledge and skills of staff within the alliance partnership in the care and treatment of children and young people.

Report to the Service Manager and contribute to the development of service pathways and delivery.

Provide timely assessments for children and young people presenting with moderate to severe mental health needs, including deliberate self-harm assessments.

Arrange appropriate follow-up appointments and ensure effective signposting to relevant services.

Provide advice, support, and education to the wider professional network, including the Hub Team.

Work closely with the Mindworks Alliance, School Based Needs teams, Surrey Well Being Partnership, and other partner agencies to respond to referrals.

Conduct specialist assessments and provide treatment to children, adolescents, and their families either independently or jointly with Hub staff.

Carry out autonomous clinical assessments for young people with complex or co-morbid mental health needs.

Work within a multidisciplinary team (MDT) providing assessment and treatment for children and young people with complex mental health difficulties.

Manage an individual caseload from referral through to discharge or transfer, including medium to high-risk cases.

Complete and oversee initial assessments and facilitate warm handovers or referrals to appropriate services.

Participate in the Hub duty rota and respond to queries from families and the wider professional network.

Develop crisis and contingency plans with young people and families, including risk assessment and positive risk-taking approaches.

Provide mentorship and guidance for students and support their learning objectives in collaboration with the Clinical Placement Facilitator.

Maintain oversight of the Hub duty rota and provide support to junior colleagues when required.

Work with the Service Manager to ensure effective clinical governance processes and quality assurance of care.

Communicate sensitive and complex information to young people and their families in a compassionate and appropriate manner.

Provide consultation and professional advice to other professionals within the CYPS division.

Ensure outcome measures are implemented, monitored, and audited across the team.

Contribute to research activities and promote evidence-based interventions.

Maintain up-to-date knowledge of safeguarding legislation, child protection procedures, and relevant policies.

Lead the development and implementation of clinical guidelines and protocols within the service.

Qualifications

- Post Graduate level qualification and registration with NMC.
Experience

- 3 years post qualifying experience, 1 of which must have been a year as a Band 6 in community. Experience working with CYP with mental health needs in the community
- Undertaking mental health assessments, risk assessments, risk formulations, crisis and contingency planning and use of outcome measures.
- Working with other agencies
- To oversee, participate and to support staff in navigation meetings, multi-disciplinary meetings, formulation meetings, risk support and safeguarding.
- Training others through various mediums
- Understanding of family functioning.
- Knowledge and awareness of Safeguarding procedures
- Experience of developing a plan of care to safeguard an adult or child
- To investigate and respond to complaints as directed by the Service Manager and Senior Leadership Team.
- Working in partnership with the Mindworks Network consulting and planning care to meet needs in line with The IThrive Framework
