  • Community Nursing Health Care Assistant, Band 3, West Berkshire, Newbury 

  • Weekend Working only! 

  • West Berkshire have an exciting position for a motivated, conscientious, and experienced Health care assistant within Newbury Community Nursing services 

  • The Community Nursing service offers care to patients in their own homes, 7 days a week, 08 .30 – 19.00, inclusive of Bank Holidays 

  • The role of Band 3 Healthcare Assistant is fundamental to the successful running of Community Nursing Teams. The post holder will be able to demonstrate the competencies required to perform at this level, role model the Trust’s core values as well as exhibit the behaviours required as part of those values 

  • The successful candidate will work across health and social care teams, playing an active role as part of a multi disciplinary team. He/she will provide planned care with proximal supervision from a registered practitioner to deliver a seamless service to patients 

  • You will be required to proactively work with other team members embracing case management and the long-term conditions agenda thus preventing avoidable hospital admissions and facilitating appropriate earlier discharges into the community 

 

Main duties of the job

  • Deliver clinical and therapeutic care to patients within agreed trust policies/ protocols, as documented in the band 3 competency assessment tool 

  • Contribute to the holistic assessment and management of patients care needs through initial data collection, monitoring of patients progress, feedback discussion reports and in care reviews with the Community Nursing Team 

  • Implement and evaluate nursing plans in response to identified needs of the individual, carers and community in partnership with clients 

  • To provide care according to the available evidence of best practice, under the direction of a registered nurse following the BHFT delegation policy 

  • To identify and plan preventative measures and anticipatory care needs in discussion with registered nursing staff 

  • To work on own initiative and within scope of competence, referring appropriately to the Community Nursing Sister and/or Community Matron for patients that may require onward referral

Detailed job description and main responsibilities

The “must haves” for this role: 

  • Full UK driving Licence 

  • Grade 9-4 (A-C) GCSE Maths and English – or functional skills level 2 in Maths and English 

  • Care certificate 

  • Preferable NVQ 3 in health and social care

  • Educated to GCSE standard in English and Maths as a minimum
  • NVQ 3 or willingness to undertake

  • Able to work in challenging environments
  • Work between the hours of 8.00am-8.00pm 365 days per year including bank holidays and weekends
