West London NHS Trust  provides a full range of mental health, community  and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.

We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

An exciting opportunity has arisen for suitably qualified and experienced candidates within our new Complex Psychosis teams across the Boroughs of Ealing, Hounslow, and Hammersmith and Fulham.  Each borough is mobilising these new services bringing together the existing Community Rehabilitation teams alongside new investment to develop an assertive outreach pathway for service users who need an assertive approach to engagement as part of their clinical presentation to provide care, treatment and support.

 Successful candidates will show knowledge and experience of the clinical processes and treatments that support the care of people experiencing psychosis with complex needs. They will also demonstrate experience of working within a multi-disciplinary team setting; taking responsibility for assessing need and setting up robust care plans to support service user recovery. They will be able to take a flexible, robust and persistent approach to working alongside service users and their support networks to deliver care and get the greatest benefit from treatment.

 These are new posts that will be available across all of our local boroughs; Ealing, Hounslow  and Hammersmith & Fulham.  When applying, could applicants indicate whether they have any preference as to the borough in which they work.

The post holder will be working in a Complex Psychosis Team within the Assertive Outreach Pathway, taking responsibility to assess, plan and coordinate the care plan and treatment of a defined caseload of service users with complex needs related to psychotic illnesses.

The Complex Psychosis Teams are a multidisciplinary, community-based service responsible for providing on-going holistic assessments, treatment and support to clients with severe and enduring mental illness living in the community. The service will enable patients with severe mental health problems to be supported and cared for in the community in the least restrictive setting through an assertive, personalised and integrated approach to care and treatment.

The service will provide an intensive community-based mental health service delivered in service users’ home environments and in supported living accommodation or other locations agreed with the service user utilising “A whole system approach” to recovery from mental ill health. The approach maximizes an individual’s quality of life and social inclusion by supporting engagement, encouraging their skills, promoting independence and autonomy in order to give them hope for the future which leads to successful community living through appropriate support

Qualifications & Training

  • Current professional qualification in Mental Health Nursing/Social Worker/OT
  • Registration must be live with a professional body such as NMC or other relevant body
  • Evidence of post registration training and CPD.

  • Training in Psychosocial Interventions or Thorn
  • Family Therapy/Work training.
  • Other relevant training, e.g. substance misuse

Experience

  • Experience of working in multi-disciplinary mental health teams with clients with severe and enduring mental health problems
  • Knowledge of CPA process and the relevant legislation that underpins mental health care.
  • Excellent verbal and written communication skills, including report writing and presentations / IT skills.
  • Ability to assess, plan, implement and evaluate a comprehensive care package for the identified client group, using a wide range of resources
  • Able to develop a comprehensive programme of intervention and support for the identified client group and their families
  • Skills in risk assessment and management.
  • Can demonstrate understanding of psychosis and its impact on service users and carers.
  • Demonstrate knowledge of current legislation and guidance applicable to this client group
  • Can demonstrate understanding of the needs of diverse groups, e.g. residents of inner city; black and minority ethnic groups, and skills in engaging with them.
  • Skills in psychosocial interventions
