The PEEER youth Service is a well established health-based youth work provision offering an expansive service to young people living with a long-term health condition across the Wessex Region.
We are recruiting an experienced youth worker to support young people living with a Kidney condition. This speciality is particularly varied, with young people from across the whole of the region accessing support as the local paediatric nephrology centre.
Young people living with a long-term health condition often face additional challenges and encounter difficult experiences whilst also missing out on many of the key events that inform and support their development as a young person. A youth worker is central to helping the young person navigate this journey and still finding a way to reach their full potential and build on their aspirations.
The PEEER youth service prides itself on a youth-led approach involving young people in the development of new programmes and shaping services to met recognised need. Youth workers will deliver online programmes, facilitate emotional wellbeing groups and lead community projects, alongside intensive 1 to 1 youth interventions to their allocated caseload either in person or virtually.
This role requires someone able to work both independently and within a team, applying the ethos of youth work within a health care setting, and ensuring young people remain central to their journey both in and out of hospital.

-To provide support for young people 11yrs - 25 (yrs.) living with a long-term kidney condition.
-To participate as a professional member of the healthcare team and contribute to the care of the patients as part of the multidisciplinary team.
-To act as a social prescribing link worker and enable access to local community services for appropriate recreational, social, educational and advocacy support where appropriate/on request of young person.
-To contribute to the provision of a varied programme of activities and opportunities to engage young people, delivered flexibly in partnership with the wider PEEER Youth Service team.
- To play a central role in developing and delivering transition from Paediatrics to Adult health support, ensuring best practice guidance is observed with collaborative working between teams. - The successful candidate must have a Professional Youth work Qualification/Youth Work related Degree or equivalent and relevant experience within Youth work.
- Experience of working within a health care setting is desirable but not essential.
- The role will require travel to accommodate young peoples needs and flexible hours with some evening and weekend work. This is a hybrid role with some working based at home, as well as community, clinical and hospital locations.

Qualifications, knowledge and experience

- Professional Youth work Qualification/Youth Work related Degree or equivalent and relevant experience within Youth work
- Evidence of continued professional development
- Significant experience working with young people
- Good understanding of health issues relating to young people
- Good understanding of social needs of young people
- Knowledge and understanding of issues relating to child protection and cultural and diversity awareness
- Knowledge of issues relating to consent and confidentiality
- Knowledge of issues relating to adolescent care
- Understanding of social issues surrounding young people with life changing illness
- Ability to support young people within a clinical environment, occasionally when undergoing treatments and medical procedures.
