You will be part of a fast-paced and continually evolving Community Multidisciplinary Team (MDT), where your ability to make high-level clinical decisions will play a key role in supporting patient recovery and preventing unnecessary hospital admissions. This will involve working closely with patients' GPs, community services, and other healthcare professionals across primary and secondary care.

You will provide clinical leadership within the Urgent Community Response (UCR) and Hospital at Home teams, delivering high-quality, patient-centred care to support individuals with acute and complex health needs in their own homes. The aim of the service is to prevent avoidable hospital admissions, facilitate early supported discharge, and promote recovery within the community.

The role includes responsibility for the day-to-day coordination of service delivery, including the management and prioritisation of patient caseloads, ensuring timely assessments and interventions, and supporting effective multidisciplinary working. You will supervise, support, and develop team members, ensuring appropriate staffing levels and skill mix to maintain safe and effective care.

You will be expected to work autonomously, undertaking comprehensive clinical assessments, risk assessments, and complex decision-making, while seeking support and guidance from colleagues and specialist services when required.
