West London NHS Trust provides a full range of mental health, community and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.
We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

An exciting opportunity has arisen within the Ealing Community Team for People with Learning Disabilities within the Learning Disabilities Nursing team. The CTPLD is looking for a Band 5 RNLD to join the team, to support the assessment and case management of people with a Learning Disability.
The successful candidate will be responsible for implementing the nursing model to assess, plan implement and evaluate care, ensuring people’s physical health needs are met; to support mainstream health services to meet the needs of people with learning disabilities and to support hospital admission and discharges.
You will work alongside the multi-disciplinary team including psychiatry, nursing, psychology, PBS, speech and language therapy, occupational therapy, physiotherapy to ensure a holistic approach is taken to manage health needs.
The successful candidate will work closely with external colleagues; social services, hospital liaison, acute physical health services and other stakeholders.

Supporting mental capacity assessment and best interest meetings
Supporting family members, carers and providers
Health promotion
Health facilitation
Implementation of care pathways
Support service users to manage their physical health needs:
Supporting GP’s to increase annual health check uptake and quality
Facilitating blood tests via desensitisation programmes
Epilepsy care plans
Supporting the dementia pathway
Symptom management and end of life care planning
Supporting weight management
Promoting positive sexual health
Promoting healthy lifestyle choices
Partnership working with the MDT:
Attending weekly referral meetings
Attending weekly case discussion meetings
Raising safeguarding concerns
Reporting incidents
Management:
Supporting student nurses in the role of practice supervisor and assessor – additional training available
Attending regular supervision
Adhering the NMC requirements inc. revalidation
