Job overview
To deliver high quality care across acute and primary care services, promoting care closer to home. To enable people with diabetes to live healthy and independent lives.
To support and promote self-management of diabetes and avoidance of ill-health and complications. To support the diabetes nursing team in all aspects of diabetes care.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
- Clinical and administrative support to the diabetes nursing team.
- Maintaining and interrogating databases.
- Liaising with all members of the MDT, including external agencies.
- To support delivery of education to HCP’s, patients and carers.
- To teach clients to glucose monitor and inject insulin in accordance with agreed procedures.
- Communicate effectively through contributing to MDT discussions, being non-judgmental.
- Keep accurate records.
- Utilise available technologies to enhance self-management of diabetes
