Job overview
We are seeking a Community Specialist Practitioner (CSP) to join our Urgent Community Response (UCR) pathway in our South Bristol integrated Network Teams (INTs)!
We want an experienced nurse, physio or paramedic who is used to working creatively to assess, diagnose and implement treatment plans for people who require crisis intervention within a community setting. You will work alongside clinical staff to manage risk for these patients in the community.
Your medical knowledge will help to reduce hospital admissions and contribute to effective admission avoidance when service users are in acute need. The post holder will be expected to contribute to service improvement initiatives.
You will be part of the point of clinical escalation rota in the South Bristol locality, using virtual assessment skills to support our staff members who are face-to-face with deteriorating or acutely unwell patients.

You will be become a part of our Integrated Neighbourhood Teams working out of the Withywood Centre and Knowle Clinic and covering the whole of South Bristol.
Due to the qualification criteria for this role requiring a Non-Medical Independent & Supplementary Prescriber (or willingness/ability to work towards) , we are currently unable to accept applications from Occupational Therapists.
Main duties of the job
As Clinical Specialist Practitioner (CSP), you will work autonomously as part of the UCR pathway in the INTs.
The role involves effective history taking, clinical assessment, diagnosis, and implementation of a range of interventions for patients presenting with acute undifferentiated and undiagnosed conditions and acute management of chronic long-term illnesses. You will be able to demonstrate effective management of clinical risk within the community to support patients to remain in their own home.
You will be able to prescribe and review medication appropriate to the needs of patients, within your scope of practice and in accordance with national and local protocols. You will oversee patient caseloads, provide senior clinical leadership for more junior staff and offer wider input into clinical leadership within our locality. We have a wonderful team of CSPs - both Urgent and Long Term Conditions with whom you will work closely alongside Operational Management to support the delivery of effective and safe clinical standards of practice.
If you are successful you will need to demonstrate critical thinking in the clinical decision-making process; identify gaps within the service; and contribute to ongoing service improvement and development in line with national and local strategies.
You will be required to work collaboratively with other members of the multidisciplinary team to establish a whole system approach to managing health care for individuals with complex care needs.

Qualifications & Training
Essential criteria
- Have or willingness to work towards Physical Assessment and Clinical Reasoning (PACR) (or equivalent)
- Have or willingness to work towards Pathophysiology and Diagnostic Reasoning for Advancing Practice (PADRAP) (or equivalent)
- Have or willingness to work towards Non-Medical Prescribing (NMP)
- Current registration with NMC / HCPC or other relevant professional body of registration
- vidence of continuous personal and professional development and commitment to undertake level 7 masters academic learning that supports the skillset required for this role
Desirable criteria
- Coaching / Teaching and Assessing qualification / experience.
Skills, Knowledge and Experience
Essential criteria
- Experience of emergency/urgent care or community setting which has required complex individual assessment and onwards planning.
- Experience of using clinical examination and diagnostic reasoning skills in an urgent setting.
- Experience of chairing meetings, for example MDT’s.
- Experience of clinical risk assessment.
- Experience of managing and leading people.
- Experience of effective supervision of junior staff and students.
- Evidence of up-to-date clinical skills relevant to the role.
