An interesting and exciting permanent  position for  band 6 RGN to join the Continuing Healthcare team in the West Area covering the localities of Gwynedd and Mon.

Fixed term staff  to join the Continuing Health Care team in West area,  Gwynedd / Mon. The post holder will manage on behalf of the West division the implementation, monitoring and reviewing of NHS Funded Registered Nursing Care (FNC) and Continuing NHS Health Care (CHC) in Care Homes (Nursing) and in the domiciliary setting to ensure patients receive care of a quality which meets best practice and national standards as prescribed and funded by the NHS

 

 

 

 

Main duties of the job

To advise each care home on improvements within the physical environment and how it can be made more dementia friendly, focusing on inexpensive techniques.

To promote high quality, cost effective, evidence based care with regards to Dementia care across North Wales. This involves the post holder having highly developed clinical skills, and a high level of knowledge, understanding and experience of a broad range of clinical and professional issues in order to meet key responsibilities.

To develop positive relationships between Primary Care, and the Independent Sector.

To provide an educational role to enhance the skills of staff within the independent sector which will then impact on the primary care setting as a whole.

 

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
