THIS POST IS FIXED TERM/SECONDMENT FOR 4 MONTHS DUE TO MEET THE NEEDS OF THE SERVICE.

The Postholder will manage on behalf of the implementation, monitoring and reviewing of NHS funded Registered Nursing Care (FNC) and Continuing NHS Health Care (CHC) in care homes (nursing) and in the domiciliary setting to ensure patients receive care of a quality which meets best practice and national standards as prescribed and funded by the NHS in line with the National Framework for Continuing Health Care.

Main duties of the job

The post holder will be expected to formally coordinate the bringing together of MDT members to undertake assessments for CHC funding within the acute, community hospitals and domiciliary setting which is consistent with national guidance and local specification (standard operating procedure) for CHC Funding. The post holder will work in partnership with the local authorities and the independent sector to complete joint reviews for CHC and FNC patients wherever possible. The post holder will provide professional nursing advice to the staff in the care homes (nursing) to ensure that appropriate referrals are made to other members of the clinical team.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
