Job overview
Are you a highly skilled and motivated clinician ready to take your career to the next level? We are seeking an experienced Advanced Clinical Practitioner (ACP) to join our dynamic multidisciplinary team and help shape the future of patient care. This postholder will work within the Community Mental Health team for Adults providing assessment, care and treatment to patients who live within the Derwentside/Stanley and Chester Le Street area.
About the Role
As an ACP, you will work autonomously across professional boundaries to assess, diagnose, treat, and manage patients with complex needs. You will be a key leader in clinical decision-making, service development, and quality improvement initiatives.
About You
- Registered healthcare professional (NMC).
- MSc in Advanced Clinical Practice or equivalent.
- Proven experience in autonomous clinical decision-making.
- Passionate about patient-centred care and service innovation.
Main duties of the job
Key Responsibilities
- Deliver high-quality, evidence-based care across the four pillars of advanced practice: clinical, leadership, education, and research.
- Undertake comprehensive assessments and formulate management plans.
- Prescribe medications and interventions within your scope of practice.
- Lead service improvement projects and mentor junior staff.
- Collaborate with colleagues across primary, secondary, and community care settings.

Qualifications/Experience
Essential criteria
- Current registration with the Nursing and Midwifery Council.
- A master’s degree in Advanced Clinical Practice or A master’s degree in a clinically relevant subject with e-portfolio accreditation.
- Relevant preparation as a mentor, practice assessor or practice supervisor or teacher or relevant experience of teaching and assessing learners in the last two years
- Substantial period of experience working in a Band 7 or equivalent role which demonstrates readiness to work at a level of advanced practice as outlined in the relevant national guidance.
Desirable criteria

- Mentoring or coaching qualifications
- Clinical supervision qualification or study
