Job overview

Are you an experienced Community Nurse who would like to join our dynamic team? We are seeking an ambitious, motivated, compassionate and experienced part time Band 6 nurse to support the changing face of community health care. As part of the Rapid Response Team who support the Urgent Community Response, Same Day Unplanned Care and Virtual Ward pathways, your role will be essential in providing efficient, integrated, urgent care to our patients with care and respect.

Main duties of the job

You will be working as part of the Rapid Response team predominantly on Unplanned Care to triage and assess referrals into the service; You will be accountable for initial history taking, clinical assessment, triage and onward referral. There is an expectation that with demand in planned care you would be flexible to cross cover.

Through face to face patient visits you will undertake a full holistic assessment, plan care with the patient at the centre and undertake physical examination of the patient with undifferentiated conditions.

You will be working closely with other members of the MDT and partners to formulate a management plan that will support the patient to remain in the community.

Part of this role will include covering our EEAST Ambulance Stack as required.

Detailed job description and main responsibilities

We are looking for an enthusiastic and motivated nurse with community experience, who is passionate about delivering high quality care.  Excellent communication, time management skills and sound clinical knowledge, is essential.  A flexible approach is required to accommodate the changing needs of the service along with the drive to influence service development.

The Trust aims to recruit and retain the best possible staff, who will make a positive contribution to the service, in return we offer excellent development opportunities and training that can be directed towards your needs/goals. You will have regular supervision, mentorship and support from within the Rapid Response Team and the Trust.

You must have a driving license and have access to a vehicle in order to carry out your duties.  You will be required to work a shift pattern covering 7 days a week from 08:00-20:00.

Essential criteria
  • Registered General Nurse
  • Demonstrate evidence-based practice and up-to-date clinical skills
  • Relevant teaching/mentoring qualification and evidence of teaching skills
Desirable criteria
  • NMP

Experience

Essential criteria
  • Significant/recent experience working at a high level (Band 6 or above)
  • Experience in triage
Desirable criteria
  • Community Experience

Knowledge

Essential criteria
  • Ability to communicate effectively
  • Ability to remain calm whilst working under pressure
