Job overview
An exciting opportunity has arisen for a fixed term 12 month contract for 30 hours per week to support the Prostate Pathway. The role of the Prostate Support worker is to work closely with clinicians and Service Managers to ensure that the suspected cancer pathway patients are effectively managed. The post holder will be required to act as a flexible clinical resource actively supporting the suspected cancer pathway. The post holder will have direct patient contact working with the wider multidisciplinary team to ensure that patient care is coordinated and delivered with minimum level of difficulty to our patients, in the service delivery of cancer waiting time pathways and targets.
Main duties of the job
Ensure that Tracking of patients is efficient and responsive, dealing with queries and day to day issues. Including the tracking of Appointments, MRI dating and reporting and monitoring of histology.
Support in data collection and audits
Alert the clinical team on MRI reports and biopsies results so they can be actioned by the clinical team in a timely manner.
Deal with queries from patients, their carers, general practitioners and members of the admin & clerical team with support from the clinical team.
Safety supporting biopsy equipment set up whilst in clinic (Training will be provided)
Maintain stock control where applicable with support of Advanced Nurse Practitioner for Prostate(ANP)
Proactively support and coordinate for the overall needs of patients on the pathway, using own judgement and escalating to the relevant practitioner as necessary. Contact patients offering them a choice of appointment dates and agree with them the date of admission for their biopsies

Detailed job description and main responsibilities
To have a sound understanding of 18 week and cancer waiting time targets, rules and procedures, and to work proactively to ensure that all targets are maintained.
Support with infoflex and active surveillance programme, including administrative duties printing of letters and PSA blood forms.
To schedule outpatient appointments ensuring that clinics are utilised appropriately and effectively.
To establish and maintain strong communication links with the clinical team
To monitor the quality of patient pathways using Trust IT systems and act to improve the quality of information within the Directorate.
Ensure Trust systems are updated with patient pathway status information and that data quality is maintained.
Liaise with internal and external colleagues to share patient pathway information and diagnostic information, and expedite patient journeys where needed.

Qualifications
Essential criteria
- • GCSE English and Mathematics (Grade A-C)
- • NVQ level 3 or 4 or equivalent experience
- • Experience of working in an NHS setting
Desirable criteria
- • Counselling qualification/experience
- • Evidence of continued role development
- • Foundation degree level training or equivalent
Experience
Essential criteria
- • Direct patient care. Experience of working in clinics and/or outpatient or inpatient setting
- • Knowledge and/or experience of working directly with people with cancer.
- • Administrative and organisational experience
