Job overview

The Community Mental Health Service (CMHS) in Bracknell has an exciting opportunity for a Band 5 Community Psychiatric Nurse to join our dynamic and multi-disciplinary team to provide advice, support and treatment to people and their carers.

The post holder will manage a caseload of clients receiving regular, injectable anti-psychotic medication (depots) and carry a small caseload as a ‘named worker’.
We are a joint funded team that provides both health and social care to support individuals aged 18 years and above with complex and severe mental illness.

The service runs Monday-Friday, 9:00am – 5:00pm. 
We are looking for dynamic candidates who are enthusiastic and passionate about patient care and recovery.

Main duties of the job

• To have clinical responsibility for the ‘depot caseload’ as specified by the Clinical Team Lead.
• To assess service user needs, plan effectively, monitor and evaluate interventions using evidence based practice which is relevant to individual needs and within a framework of research and audit.
• To participate in duty work on a regular basis.
• To establish and maintain good liaison with GP practices, and other physical and mental health services including other relevant agencies and organisations in the area.
• This post is not for usual care co-ordination but there is opportunity to begin learning about the role of the ‘named worker’ under the new CMHS model. 
• Support of nursing students.

Detailed job description and main responsibilities

1.    Degree level qualification in Mental Health Nursing and registration with NMC
2.    Recent and previous experience working with individuals with complex mental health needs
3.    Valid driving licence and access to a car.

Essential criteria
  • Degree in Mental Health Nursing
Desirable criteria
  • Specialist knowledge acquired through training in a specialised discipline

Previous Experience

Essential criteria
  • Previous knowledge and experience of working in a care environment and with service users who have complex mental health needs

Knowledge, Skills & Abilities

Essential criteria
  • Ability to administer depot medication
  • Experience of working effectively in a multi-disciplinary team and multi-agency environment
  • Knowledge and understanding of relevant Health and Social Care legislation including Mental Health Act (2025), Mental Capacity Act (2005) and the Care Act (2014)
  • Effective and competent interpersonal, influencing, communication, observation and reporting skills
  • Strong organisational skills and the ability to analyse situations and be able to make strong decisions
  • Able to empathise, be supportive and sensitive to the needs of patients as well as being able to cope with distressing circumstances
  • Ability to cope in a pressurised environment and apply practical problem-solving skills in everyday situations
  • Competent IT skills and the ability to navigate around various systems and software packages (such as Outlook, Excel and the internet)
  • Able to travel and undertake home visits as required by the job role
  • Ability to work effectively in a team, work on own initiative, work unsupervised and manage own workload
  • Manage difficult situations with service users that have complex mental health conditions and challenging behaviours
Desirable criteria
  • Experience of clinically supervising junior and unqualified staff and mentoring students
  • Substantial experience of managing a busy caseload of service users with a range of mental health conditions
