Job overview

Holistically care for own group of patients with CKD stage 4/5 preparing them psychologically & physically for renal replacement therapy. 
This involves educating patients and families on all methods of renal replacement following the patient through the decision regarding renal replacement. Working closely with haemodialysis, peritoneal dialysis & transplant teams to ensure knowledge & practice is up to date.
Supporting the  patient in the in & out patient setting – this will involve clinic & home visits.
Own patient case load.
Working within the wider Home therapy team on occasions
Anaemia Management of Kidney Care patients. 

Main duties of the job

Trouble shoot patients phone problems liaising with doctors 
Attend low clearance clinic and support doctors. Act as patients advocate where appropriate. 
Review patient notes following clinic and action 
 Attend audits and action results
Discuss pre dialysis patients at weekly MDT meeting
Refer patients to members of MDT 
Organise and see own group of patients for Pre Dialysis education and assessment in both hospital and home environment.
Ensure that patients are seen for education within a timely fashion 
Maintain contact with patients and their families towards end of life and make homes visits where necessary ensuring a log is kept for home visits for future auditing purposes.
Work autonomously and with all members of the multi-disciplinary team as appropriate
Take responsibility for own professional development
Show evidence of effective time management
Own patient case load

Experience

  • Minimum 2 years of experience in Renal
  • Must have recent clinical experience in speciality area
  • Experience in mentoring, facilitation, teaching, assessing, coaching
  • Experience of Leadership/Management and Speciality Development.
