The posts are offered as part time or full-time hours offering a fantastic work life balance, with a shift pattern which means you only work one weekend per month.
We are a friendly and supportive team, looking for highly motivated RNs who possess strong communication skills. Your role will be pivotal within the local community by making a difference to our patients, delivering a high level of care in their own home.
As a Community Nurse no day is the same and you will be met with patients of varied needs, from wound care to end of life care. Giving you the opportunity to develop coupled with a supportive environment with opportunities for further career development,
A typical week.
Monday to Friday; 8.30 - 16.30
1 weekend every 4 weeks (Time off week)
Early Shift 07:00 - 15:00 Late Shift 11.00 - 19:00

Main duties of the job

- You will visit several patients daily in their own home.
- You will provide high quality nursing care to a range of patients with varying healthcare needs.
- Provide a high standard of holistic assessment and care planning.
- You will be expected to provide wound management, health promotion, medicines management, end of life care.
- With bases at Woodley, Finchampstead & Chalfont Surgery.

Detailed job description and main responsibilities

- A passion for community nursing, that matches our passion to provide excellent healthcare for patients within their own homes
- Either a qualified adult nurse or currently in your last year of training (with NMC registration)
- Hold a full UK Driving Licence and have use of a car for work purposes

- Well-developed written and verbal communication skills

- Able to demonstrate use of own initiative and ability to work alone
