The post holder will be able to provide expert clinical expertise, operational management, to the Virtual Wards (Hospital @Home) Model within Birmingham Community Healthcare (BCHC) NHS Foundation Trust.
The purpose of the role is to work across the system in a variety of city-wide settings but not exclusive to the community and inpatient units, and to in-reach into secondary care, to identify and assess patients, to prevent avoidable hospital admissions and facilitate early supportive discharge.
The work will be undertaken collaboratively with Secondary and Primary Care, and Community Services, including Urgent Care Response, Early Intervention Team (EICT), and Specialists Services.
The post holder will work attend integrated multidisciplinary teams (MDT’s) to provide advice and support and agreed data to support the Virtual Ward (Hospital @Home) Model.
Main duties of the job
- Work in accordance with the NMC / HCPC Code of Practice
- Be conversant with all BCHC NHS FT policies and procedures and ensure they are correctly implemented

Ensure evidence-based care is practiced and promoted at all times.

Manage own time effectively and prioritise workload.

Facilitate teaching and supervision for less experienced members of staff.

Promote health by collaborative working with patients and their relatives.

Provide support to the multidisciplinary team to enable patients to make better choices for their health and that of their families, undertaking health promotion and contributing to the prevention of adverse effects to health and wellbeing.

Contribute to the review and development of integrated care pathways and clinical guidelines to ensure a standardised approach to care.

Safeguard individuals from abuse by reporting any incidents involving potential or actual abuse and acting upon concerns immediately.

Detailed job description and main responsibilities
1. Facilitate and empower staff development and empowerment of all team members to perform to high standards and to innovate.

- Registered Nurse Level 1 (Adult)/ AHP.
- Advanced Decision Making
- Independent Prescribing
- Mentoring or teaching qualification.
- Evidence of up-to-date post registration education relevant to the role.

Experience
Essential criteria
- Significant experience as a registered health professional working in ED, frailty, community or older adult setting or care
- Significant experience of a multidisciplinary approach to care.
- Experience of working in a telephone / remote triage service
Skills/knowledge
Essential criteria
- Skills to organise team workload and prioritise.
- Awareness of legislation affecting the role including equal opportunities law.
- Ability to evidence application of utilising evidence-based practice. Work autonomously
- Comprehensive assessment skills and knowledge of the needs of the older person including risk management.
- Knowledge of current legislation, particularly those affecting community services.
- Computer literacy skills.
- Knowledge and application of the NMC/ HCPC Code of Practice.
- Understanding of the principles and application of Clinical Governance
- Excellent verbal and written communication skills.
- Understanding of the complex needs of a socially and culturally diverse population.
