Job overview

The post holder will work as part of a team to participate in the delivery of health care and promote the health and welfare of young adults attending Special Colleges and to acknowledge and respect the individual rights and personal dignity of the students. The post holder will work autonomously under the guidance of the band 6 nurse within their sphere of professional practice, working within teams and service guidelines and objectives. To be involved in the assessment, planning, implementation and evaluation of individual care needs in line with the core offer of service. To take charge of a designated area in the absence of the band 6 Special College Nurse. To support the teaching of competencies to education staff and support staff. To ensure high quality core offer from the Special College Nursing Service consists of:

  • assessment and review
  • hands on clinical care
  • key worker and signposting including transition
  • training and supervision, and
  • a general role in optimising long term health and lifestyles

 

Main duties of the job

·         As designated by the band 6 Special College Nurse, you may be required to work at one or more Special Colleges

 ·         To assess care needs, plan, implement and evaluate programmes of care.

·         To carry out prescribed nursing interventions as per the care plan under the direction of the Special College Nurse.  Each care plan will identify:- 

o        Existing clinical needs and how and by whom these will be met.

o        Monitoring the child’s growth and development in line with the Healthy weight programme.

o        Progress against the plan, expected outcomes and timescales.

o        Safeguarding needs.

o        The care plan will be made available to complement the young adults’ Education Plans. 

·         To prioritise daily care, responding to emergencies as required. 

·         To work with the Special College Nurse to assess the needs of the college population and promote healthy lifestyles within the Public Health Role of School Nursing. 

·         To ensure safe custody and administration of prescribed medication in accordance with Trust and NMC guidelines.

Detailed job description and main responsibilities

 

·         To liaise appropriately and effectively with education staff, other agencies and families/carers to achieve optimum health of the young adults. 

·         To demonstrate, teach and supervise aspects of nursing care that can be delegated to support staff and education staff. 

·         To contribute to the induction of new staff. 

·         Participate in the prevention, identification and protection of adults at risk in accordance with Trust Safeguarding Procedures. 

·         To facilitate and undertake nurse led assessments where relevant. 

·         Undertake screening programmes in accordance with Trust Policy. 

·         To provide advice to parents, pupils and school staff on preventative measures to limit outbreaks of infectious diseases 

·         Observes own personal duty of care to equipment and resources.

Qualifications/Education

Essential criteria
  • Educated to the following level: • Relevant professional qualification – RN Adult Branch or RNLD
  • Evidence of continuous professional development

Experience

Essential criteria
  • Post registration community experience
  • Evidence of multi-disciplinary working
  • Experience of working with children or adults with profound and multiple learning disabilities.

Skill/Knowledge

Essential criteria
  • Sound assessment skills and use of nursing process and models as relevant
  • Sound knowledge and application of NMC Code of Conduct and Scope of Practice
  • Ability to teach others how to carry out clinical care procedures as required and support others with competency documentation.
  • Competence in undertaken both enteral and Nasogastric feeding
  • Good time management skills
Desirable criteria
  • Understanding of current changes affecting the NHS and Social Care, particularly in relation to Primary Care
  • Understanding of Equality and Diversity and awareness of and sensitivity to the needs of those from minority ethnic groups, including adults with disabilities and/or additional health needs
  • Evidence of ability to participate in audit
  • Demonstrates individual responsibility in respect of Health and Safety
