Job overview

An exciting opportunity has arisen for an enthusiastic Registered Nurse to join Dudley Community Diabetes Specialist Nursing Team.  The team works closely with the Acute Team as part of a specialist MDT approach to managing Diabetes.

The successful applicant will have excellent communication/interpersonal skills/clinical skills & be able to demonstrate the ability to work & make decisions autonomously within a multi-disciplinary team which includes consultants, GPs, dietitians, podiatrists & other specialist services.

The Diabetes Specialist Nurse will work closely with the local diabetes population delivering services in patient’s homes, clinics & care homes. They will be expected to manage their own caseload effectively to deliver specialist diabetes care as well as presenting diabetes structured education.  

The successful applicant will have a valid UK driving licence and access to a vehicle with business insurance.  They will be expected to support the Lead Nurse in the continued transformation of service delivery. 

Main duties of the job

The postholder will:

  • Work as part of the diabetes specialist nursing team, supporting consultant diabetologists & wider Trust staff in the management, care & education of the patient with diabetes
  • Be responsible for specialist assessment, providing support & education to patients with diabetes/ their families/ carers ensuring a high quality service is provided
  • Be a member of the multi-disciplinary team & exercise a high degree of personal & professional autonomy utilising skills/knowledge/ assessment in decision making
  • Contribute to service improvement in the clinical field monitoring & improving standards of care in accordance with the local & national policies & NICE guidance
  • Liaise with other members of the diabetes team and Diabetes Nurse Specialists in Dudley
  • Support the Lead Nurse, Matron & Medical Head of Service in developing & monitoring diabetes services & the quality of care experienced by patients with Diabetes

Education/Qualifications

Essential criteria
  • First level registration
  • Evidence of undertaking a recognised course of study in teaching and assessing
  • Educated to 1st degree level or evidence of study at degree level.
  • Evidence of professional development and regular study since qualification
Desirable criteria
  • Up to date practice assessor certificate/evidence of being on the Live Mentorship Register
  • Post registration experience working in an acute and community setting
  • Non medical prescribing

Experience

Essential criteria
  • Must have recent nursing experience as a senior staff nurse or above within a diabetic related environment
  • Experience of regularly managing a team of staff and other resources during shifts, within the acute setting
  • Experience of close liaison with members of multidisciplinary team

Knowledge and Training

Essential criteria
  • Knowledge of current clinical development, evidence based practices and professional issues affecting nursing and diabetic services.
  • Commitment to evidence based practice
  • Keen to develop own skills and develop nursing practice.
Desirable criteria
  • Developed and implemented safe systems of work to ensure health and safety of staff, patient and visitors
  • Understanding of leadership theory and practice.
  • Evidence of having implemented teamwork practices.
  • Knowledge of effective people management practices
  • Knowledge of clinical audit and effectiveness systems.
  • Projects/research undertaken on own initiative or as part of previous nursing posts held
  • Willing to undertake specialist diabetes competencies.
