This is an exciting opportunity for a qualified school nurse or health visitor who has the transfer fields of practice or is willing to undertake this course or experienced staff nurse.
The role is part of our Inclusion pathway within the Healthy Child Programme Service 0-19 which incorporates Health Visiting, School Nursing and Teenage Pregnancy Services. It is an exciting time to join our service and be part of developing and shaping services for babies, children, young people and their families.

Main duties of the job
The Senior Practitioner (School Nursing) - Youth Justice post sits within the Inclusion Pathway HCP 0-19 and is part of the School Nursing provision. The post holder will lead on the provision of the Healthy Child Programme (HCP) to children and young people who are supported within the Youth Justice Service. The post holder will work closely with external colleagues within the Youth Justice Team and internal colleagues delivering care as part of the HCP 0-19 Inclusion Pathway
The School Nursing, Health Visiting and Teenage Pregnancy form the Healthy Child Programme 0-19 which deliver public health services to babies, children and young people and their families. The overarching aim of public health services is to prevent, promote and protect the health and wellbeing of children and families. They work with stakeholders across a number of settings and organisations to lead delivery of the Healthy Child Programme (5-19) to improve child health outcomes and ensure that families in need of help or at risk are identified at the earliest opportunity.

Detailed job description and main responsibilities
- To manage your own caseload, identifying and responding to priorities. To plan, develop, implement and evaluate programmes of care.
- Contribute to the development, delivery and evaluation of health education programmes and staff training
- Be able to work in a single point of access to assess new referrals, manage risk, provide parent and professional consultation and provide information to other agencies, in line with information sharing agreements.
- To provide clinical advice and support to the team, provide caseload supervision, 1-1’s and PDR’s for junior staff. Provide learning opportunities, induction, and preceptorship for all learners.
- Contribute to the development of, implement and evaluate new and existing policies ensuring that practice is evidence-based.
- Maintain clinical governance processes, e.g., identification & management of risks and incidents
- To co-ordinate and/or contribute to the health needs assessment of children under the care of the team, participating in the multi-disciplinary and multi-agency assessment & intervention of/for children & young people.
- To provide clinical advice and support, on aspects of care for children & young people.
- Take the lead professional/key worker role for children & young people when appropriate and work as part of multi-agency teams to support those on child protection, child in need and early help plans or those who are looked after.
- To advocate for families within the multidisciplinary team and make referrals as appropriate.
- To work closely with members of the youth justice service, schools and education provisions and provide advice and support to parents and staff where children or young people have identified health needs.
- To have a wide range of knowledge in approaches to communicating and managing the care of children, young people and their parents/carers.
- To be able to effectively communicate with colleagues, peers, senior managers and clinical leads within the Trust.
- To be able to communicate complex patient related information facilitating positive outcomes and ensuring collaborative working.
- To act up in the absence of the SCPHN.
- To always practice within NMC standards and the Code of Conduct

Qualifications
Essential criteria
- Registered Children’s Nurse – Child Branch/RSCN/ SCPHN (school Nursing)
- Additional training (or equivalent demonstrable experience and in working with children and young people delivering the Healthy Child programme within a School Nursing team
- Evidence of additional qualification or demonstratable of development of knowledge/skills through experience in a specialist area of support e.g., young carers, medical needs, asylum seekers, continence, emotional health & wellbeing.
Experience

- Demonstrable experience of working with children and young people in school nursing service
- Demonstratable experience of multidisciplinary working In e.g., safeguarding and Early Help agenda’s
- Demonstrable of managing your own caseload and delivery of the wider HCP 5-19 programme
Knowledge and Skills
Essential criteria
- Legislation and guidelines relating to children and young people, including Safeguarding and prevent.
- Aware of own abilities and be able to carry out clinical role without continuous direct supervision and ability to supervise junior members of staff
- To understand, identify and act upon changing clinical needs of children and young people
- Knowledge and ability to maintain robust clinical governance to deliver safe, high-quality service
- IT knowledge & skills to be able to maintain databases, operate electronic systems, produce reports.
- Communication skills both verbal and written when interacting with members of the team, other professionals, clients and their families
- Demonstrates the ability to work effectively within a multi -disciplinary team
- The ability to deliver sensitive information to families in a way that considers their level of understanding
- Ability to respond appropriately to children and young people in need and/or children at risk in accordance safeguarding guidance
- Evidence of supervising other team members and working on own initiative and prioritising workload
