The Royal Wolverhampton Continence Care Service are highly motivated and passionate about providing high quality, evidence-based care to

exceptional individuals with a passion for delivering a high-quality service with a focus on patient centered care.
You must be a Registered Nurse, with experience of working autonomously within a community setting. Experience of assessing, implementing, and evaluating care for people with continence related disorders is essential. You will have proven skills/experience in leadership mentorship and management to include implementing change.
You will be skilled and experienced in using NHS computer systems to include Microsoft office and be confident utilizing applications to enable
remote consultation with patients/to attend MDTs.

The service work closely with members of the wider multi-disciplinary teams including local Hospices, Urgent care team, Care Coordination Hub, Ambulatory Teams and Support Care Teams, Acute Services, Primary Care and Care Establishments. Car Owner and driver essential.
Main duties of the job
The opportunity has arisen for a highly motivated individual to join The Wolverhampton Continence Care Service. The successful candidate will need to demonstrate a proven interest in continence with relevant post registration experience.
You will be a motivated, compassionate and flexible Registered Nurse with excellent communication skills. You will be responsible for providing
assessment and treatment programs to patients with complex bladder and bowel problems. Our service provides a range of Clinic,
telephone and home patient contacts. All members of our team actively participate with ongoing service improvement and quality improvement
programs.
Experience of community working is required for the role as you will hold your own caseload and use a multi-professional approach to ensure
patients receive clinically proven and cost effective treatment. You will also be required to provide education to health care professionals, patients and carers.

Detailed job description and main responsibilities
Clinical Care & Patient Management
- Deliver high-quality, compassionate nursing care tailored to the needs of diverse patient populations.
- Autonomously assess, plan, implement, and evaluate individualized care, ensuring best practice and adherence to local healthcare policies.
- Work with patients carers and relatives to ensure that they are partners in their care.
- Adapt clinical approaches to meet the unique healthcare challenges.
- Work within the limitations of the continence budget without compromising the clinical needs of the patient.
- Recognize and respond appropriately to deteriorating patients, escalating concerns as necessary.
Leadership & Team Collaboration
- Provide clinical leadership, supporting and mentoring junior nursing staff, healthcare assistants, and students.
- Act as a role model, upholding professional standards and ensuring high-quality person centered care delivery.
- Work collaboratively within multidisciplinary teams, contributing to patient care plans and treatment strategies.
- Lead and ensure accurate documentation for clear, effective communication in accordance with the NMC Code of Conduct and local standards.
- Lead in the provision of a high standard of care that ensures that compassion, dignity and respect is maintained.
- To deputize in the absence of the Continence Manager and Senior Nurse ensuring that service continuity is maintained.
- Work harmoniously and effectively with colleagues to establish robust communication networks within the team.
Service Development & Quality Improvement
- Contribute to the development and implementation of policies and procedures that enhance patient safety and service efficiency.
- Identify areas for improvement and participate in audits, research, and quality assurance initiatives.
- Support the education and training of healthcare professionals, promoting continuous professional development.
- Implement infection prevention and control measures, ensuring compliance with health and safety regulations.
Adaptability & Cultural Awareness
- Work effectively within different venues, adapting to varying clinical practices and regulatory frameworks.
- Demonstrate cultural awareness and sensitivity when delivering care to diverse patient groups.
