Job overview
We are excited to offer an excellent opportunity for an experienced and enthusiastic nurse to join our heart failure service as a Band 6 CNS. This post offers the chance to be part of a multidisciplinary team that delivers high-quality, patient-centred care across both hospital and outpatient settings. You will be based at New Cross Hospital and play a key role in the assessment, management, and ongoing support of adults living with heart failure.
As a HF CNS, core aspects of your role will be to provide patient education, suggesting the use of heart failure treatments, creating treatment plans and arranging follow-up following both inpatient and outpatient reviews, in collaboration with senior heart failure Nurses. We work closely alongside cardiologists, GPs, physiologists and pharmacists as well as primary care and community teams.
We are looking for a Registered Nurse with post-registration experience caring for patients with heart failure. You should have relevant cardiology post-registration qualifications at Level 6 or Level 7. Clinical assessment skills are desirable however development will be supported. Strong communication skills, and the ability to work independently and as part of a wider MDT are essential.

Main duties of the job
Develop individual competence and knowledge which will support the provision and evaluation of seamless specialist nursing services, ensuring patients receive the highest standards of clinical care. You will develop specialist knowledge and skills, demonstrated through the provision of advice /education and support to staff, patients, families and carers. You will be supported by a more experienced nurse or nurses to enable you to achieve competence in agreed areas of specialist practice through exposure, training and education.
Responsible for on-going development and training of the policies, procedures, protocols and guidelines. You are accountable for safe, effective clinical practice working as part of a defined team, supporting team performance, ensuring efficient and effective use of physical and human resources, and will also provide / deliver a range of educational and training packages to health care staff and patients.
