Job overview

An exciting opportunity has arisen to become a Community case manager/ district nurse with the Amersham Adult Community Healthcare Team.

This role is part time and is 32.25 hours a week. 

The role is providing nursing interventions to patients in their home environment. If you are interested in joining our friendly team, we would love to hear from you. You must be a car driver with access to a car you can use for work.

In this role you will become a competent Community Nurse. Our skills include IV's, catheter care, palliative care and end of life support, leg ulcer management, wound care, medication management, complex care management, prevention of admission work and MDT working.

Our days are varied, with teamwork and compassion being at the heart of everything we do.  Amersham ACHT aim to provide the highest standard of care for our patients.  We will support you through your induction and gaining your skill competencies.  

Ideally you should have a wide range of clinical expertise within the NHS as well as experience working in a multidisciplinary team environment. The post holder must be able to demonstrate their professional skills and competence in medicines management, care and treatment delivery. You must be innovative in your approach and willing to work with other members of the community team and ward staff to ensure a high-quality service delivery. You must be willing to develop your own knowledge and skills. We  provide a 7am to 9pm service 7 days a week.

Main duties of the job

  • The case management model will provide interventions tailored to meet the needs of the individual 
  • To contribute to processes for effectively caseload management for   patients in community setting  who are very high risk of acute deterioration and  unplanned admission to hospital
  • Assess the physical and psycho–social needs of the defined case load instigating therapeutic treatments and care delivery based on best available evidence
  • To improve health outcomes using specialist nursing skills and knowledge
  • To undertake referral management to develop and establish case management programmes for patients referred to the Adult Community Healthcare Teams
  • To manage a caseload of patients with long term conditions who would benefit from case management

Education, Qualifications & Training

Essential criteria
  • Registered Allied Healthcare Professional / Registered Level 1
  • Nurse Degree/ Diploma in Health / Nursing Studies or related subject
  • Evidence of recent Continuous Professional Development
  • At least one year experience as a Band 5 in the community
  • Relevant post graduate clinical training e.g. Elderly, Long Term Conditions, Respiratory Disease, Cardiac Disease, Dementia, Rehabilitation, Trauma and Orthopaedics and End of Life

Skills, Abilities & Knowledge

Essential criteria
  • Willingness to undertake the District Nurse Training

Experience

Essential criteria
  • Demonstrate broad experience post registration within a variety of core areas
  • In depth experience of working within multi agency / multi disciplinary team
  • Demonstrable experience of working in a busy office and pressurised environment and managing a diverse workload
Desirable criteria
  • Experience of working in a community/in-patient setting for a minimum of 3 years post registration
  • Experience of assisting with implementing change management programmes e.g. service development
  • Experience of electronic clinical systems
  • Experience of day to day staff management e.g. case allocation to Health Care Assistants

Special Circumstances

Essential criteria
  • Valid and current driving license with access to a car for work. Driving is essential to the role and travel to locations throughout Buckinghamshire
