Job overview
An exciting opportunity has arisen to join our established and highly respected Critical Care Outreach Team. The team delivers a cross site 24/7 service at Stoke Mandeville Hospital and Wycombe General Hospital at Buckinghamshire Healthcare NHS Trust.
We are looking for a highly motivated, innovative and expert practitioner to deliver patient-focused care to acutely ill, deteriorating and critically ill patients across the trust. The successful candidate will need to display excellent communication, interpersonal and leadership skills, be able to respond to rapidly changing and high-pressure situations.
Applicants for this role need to be able to influence practice, challenge current thinking and be able to support quality improvement initiatives to improve patient safety. We are therefore looking for candidates with significant critical or acute care experience at a senior level to support the Outreach team objectives and vision.
Main duties of the job
You will be able to work autonomously and as part of a team and have good critical thinking and decision-making skills. You will be instrumental in the early recognition of deterioration and deliver an effective response, liaising with the patient’s own team.
You will be able to facilitate smooth transitions of care for patients stepping down from Intensive and High Dependency Care to the general wards and be involved in the provision of a range of education for the multidisciplinary team. As a senior member of the team, you will be responsible for providing support and mentorship to junior team members and ward staff.

EDUCATION, QUALIFICATIONS & TRAINING
Essential criteria
- Registered Nurse (Level 1)/ AHP
- Recognised mentorship/teaching and assessing course
- Evidence of further continued professional development
- Post-qualification Critical Care Course or equivalent course
- Recognised history taking and physical assessment course or willingness to undertake
Desirable criteria
- A first degree in relevant professional studies is desirable
- Master's level education or working towards
- Accredited Leadership/Management Course
- ALS provider or willingness to undertake
- Non-medical prescribing or willingness to undertake inclusive of ODPs once professional regulation allows
EXPERIENCE
Essential criteria
- 5 years' experience, including 2 years as a band 6 in ICU or equivalent Critical Care/Acute area
- Previous experience as a shift/team leader
- Experience of teaching/mentoring/ coaching of the multi-professional team
Desirable criteria
- Experience of performing an outreach role
- Experience of involvement in Quality Improvement/ successful change management
SKILLS, ABILITIES & KNOWLEDGE
Essential criteria
- Excellent personal and communication skills (written & verbal)
- Excellent IT skills, able to work with a range of software i.e. Microsoft office, Excel, Word etc
SPECIAL CIRCUMSTANCES
Desirable criteria
- Able to travel to other sites and off site as necessary

You must have appropriate UK professional registration.
