Job overview

This opportunity has risen to be part of a multi- disciplinary frailty team that provides autonomous clinical support to the care of patients undertaking frailty health assessments: diagnoses, treating, and discharging patients within their own competence.  

The Trainee Advanced Practitioner (AP) will be supported to complete Master Level studies so they will be able to provide expert care and treatment to patients within agreed protocols and clinical guidelines when qualified. However, the AP also has the ability to make decisions outside these frameworks, where necessary based on best practice. As a trainee the post holder will be responsible for an individual case load, but will have access to guidance from the assigned clinical supervisor.  

The post holder will be expected to rotate between our acute sites at Stoke Mandeville Hospital, Frailty Same Day Emergency Care (FSDEC), Frailty hospital at home team that delivers acute care in the community, community inpatient areas at Amersham and Buckinghamshire and our day unit at Wycombe General Hospital. We operate 24 hours a day, 365 days a year, and the post holder will be required to work flexibly to accommodate a developing 24hr frailty model of care.

Main duties of the job

The post holder will: 

Work towards health promotion and prevention and comprehensively assess patients for risk factors and early signs of illness 

·       Draw on a diverse range of knowledge in their decision-making to determine evidence-based therapeutic interventions, which will include prescribing medication where legally allowed and actively monitoring the effectiveness of therapeutic interventions.

·       Plan and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use, and provide direct support to patients and clients.

·       Work in partnership with patients and support them to manage and live their lives.

In addition, they will help develop a practice development programme for staff in Frailty Team and Department of Medicine for Older People, who will also be caring for these patients.

 

The Advanced Practitioner practice should be developed to an advanced level whereby the advanced practitioner can directly or indirectly influence all aspects of care and management of patients within their speciality, and across a population. This will be dependent on their level and stage of training.

Person specification

EDUCATION, QUALIFICATIONS & TRAINING

Essential criteria
  • First-level degree (HEI Level 5 or 6) or equivalent in the undergraduate base profession
  • NMC or HCPC registration
  • Specialist post-graduate education/qualifications or training relevant to the area of clinical practice for the AP role
  • Ability to study at Masters level education and meet NHSE criteria for funding
  • Ability to complete full MSc in Advanced Practice (including physical assessment module and non-medical prescribing if supported by professional regulations)

Experience

Essential criteria
  • Substantial post registration experience and three years’ experience working at band 6 or equivalent
  • Experience of working with older people and long-term conditions.
  • Experience of multi-agency / multi-disciplinary team working across service interfaces.
  • Experience of involvement in Quality Improvement/ successful change management
Desirable criteria
  • Demonstrate a high level of clinical and practical skills.
  • Experience of complex case management.
  • Experience of instigating and interpreting investigations and initiating appropriate plans of care for patients.

SKILLS, ABILITIES & KNOWLEDGE

SPECIAL CIRCUMSTANCES

Essential criteria
  • Ability to travel cross site and to community
  • Ability to work internal rotation, involving 24/7 cover.
