 North West Anglia Foundation trust (NWAFT) covers a number of key sites including Peterborough City Hospital (PCH), Hinchinbrook and Stamford, cross site working is essential.  You will develop skilled practice in the multidisciplinary management and nursing care of adult patients with a range of respiratory conditions such as Interstitial lung disease (ILS), Asthma, COPD, Bronchiectasis, Home Oxygen, and pulmonary rehabilitation. This role will be working within our hospital team, covering both inpatient wards, outpatient clinics and some home visits. You will offer an individualised service respecting the many diverse cultural and spiritual backgrounds of our service users.

 

The applicant must have the means to travel around the community, to attend patents in their own home and work for on all trust sites.

Main duties of the job

1.Be an accountable professional acting in the best interests of people, putting them first and working to providing complex clinical care that is evidence-based, person-centred, safe, and compassionate.

  • To participate in inpatient and outpatient care, provide support, information, and education to empower patients to make informed decision about their ongoing chronic disease management
  • To improve quality of life by treating patients holistically, tailoring their treatment to provide individualised evidence based, high quality care.
  • To only act within own scope of competence including Non-Medical prescribing.
  1. Use existing knowledge and work towards expertise and enhanced levels of clinical judgement to independently undertake complex and holistic assessments. 
  2. Act independently to plan, deliver, monitor, and evaluate complex care using enhanced clinical assessments, diagnostics, and interventions.
  1. Act as an expert resource within their own organisation and for external agencies, asking for advice when appropriate.
  • Review referrals and offer advice to other clinicians when required.
  • Support the delivery of education and training opportunities for others within own scope of practice.

5.Communicate effectively in challenging environments and situations with patients, their families/carers, and the multi-disciplinary team. 

6. Promote and encourage innovative clinical practice to support a culture of excellence within the wider health and care team.

Detailed job description and main responsibilities

To provide a respiratory service for patients under the care of Physicians in the Trust, and provide advice and guidance to colleagues within primary care.

To have a good understanding of national guidelines for ILD, COPD, Bronchiectasis, asthma and oxygen, including diagnosis and evidence-based interventions.

To manage and supervise the clinical care of respiratory patients including assessment, care planning, and on-going support, monitor adherence, including inhaler technique, untoward side effects of the drugs, and assess and arrange provision of oxygen therapy, assessment of functional ability (e.g. 6 minute walk test) and palliation of symptoms

To be able to provide effective smoking cessation advice and recommend interventions and support.

To provide advice, assessment and arrange the installation of oxygen therapy.

To perform diagnostic procedures such as spirometry and capillary blood gas sampling.

Education and Qualifications

Essential criteria
  • NMC Level 1 Registered Nurse/HCPC registered
  • Advance clinical assessment or willing to undertake
  • Respiratory module at degree level or willingness to undertake
Desirable criteria
  • Non-medical prescribing or willing to undertake

Experience

Essential criteria
  • Minimum of 2 years post registration within an acute or critical care setting
  • Ability to impart complex information
  • Ability to build a rapport with patients and the MDT
