THIS POST IS FIXED TERM/SECONDMENT FOR 12 MONTHS DUE TO MEET THE NEEDS OF THE SERVICE

This is an exciting opportunity for a clinical practitioner experienced in working at an advanced level ideally within primary care. With a recognisable  qualification, having completed additional qualifications minimal of degree level and independent prescribing to join a new service.

The transformation bid is a new GP triage system set in our well being service, where you will be required to triage calls from patients, carers, family and decide the best course of action applying Prudent principles. The service operates Monday to Friday 8am to 6pm excluding weekend and bank holidays.

The successful candidate will be expected to undertake telephone triage, during the daytime service, which operates Monday to Friday. A background in one or more of the following areas is desirable – Primary care, (GP practice) or acute medicine. The projects aim is to reduce the pressure on GP practices and sign post more appropriately. The vacancy is for 2.0 whole time equivalent post, candidates will be considered for full time / part time hours  - for discussion at interview. Job share will be considered.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
