Job overview

An exciting opportunity has arisen for the recruitment of a Community Continuing Care Team Sister/Charge Nurse Band 6 within Carmarthenshire community nursing service.  

We are looking for an enthusiastic and proactive nurse to provide a high standard of patient care. The ideal candidate will be able to demonstrate effective leadership, team and partnership working. Interested applicants will need to have had substantial community experience with a postgraduate qualification in a relevant course (or working towards). 

As a clinical nurse you will work without direct supervision and be accountable for the delivery of a high standard of cost-effective nursing care to those who require it in the community. You will lead on promoting the delivery of high-quality care within the Team and supporting team members to ensure a high standard of patient care.

The post requires excellent communication skills, a flexible approach to work and good time management skills. Good IT skills are essential. The post holder will need the ability to travel between sites in a timely manner.

Main duties of the job

The successful candidate will be expected to combine excellent clinical practice with day to day shift coordination and undertaking 
leadership and management responsibilities of the Carmarthenshire Continuing Care Team in Carmarthenshire.  

Provide supervisory management and strong clinical leadership skills, demonstrating emotional intelligence to recognise pressures on staff and the development of mechanisms to support and develop staff to recognise the impact of caring for people with Continuing Health Care and Primary Health need in a community environment which is complex and 
pressurised. 

Work alongside the community nursing team members, Patient Flow and wider community MDT, providing support to junior colleagues in the provision of direct care to meet the continuing health care needs for individuals, groups and the wider community.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

We have over 13,000 staff, and together we provide primary, community, in-hospital, mental health and learning disabilities services.

  • Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest 
  • Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire 
  • Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings 
  • 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers 
  • Numerous mental health and learning disabilities services

Qualifications and Knowledge

Essential criteria
  • Registered Nurse (Part 1)
  • Current NMC Registration
  • Bachelor degree (or equivalent experience)
  • Pertinent postgraduate diploma (or equivalent development/ experience and a willingness to undertake appropriate post graduate diploma level study)
  • Understanding of wider nursing and health care related agenda
Desirable criteria
  • Teaching qualification (or equivalent experience)
  • Recognised management qualification
  • Community Nurse Prescriber
  • Specialist Practice Award in District Nursing
  • Bronze IQT

Experience

Essential criteria
  • Appropriate experience within community/specialty and able to demonstrate sound knowledge
  • Able to demonstrate sound knowledge of NMC Code (2015)
  • Experience of working in a multi-professional environment
  • Experience of running a ward/area/department
  • Experience of working with preregistration nursing/ midwifery students
  • Experience of formal/informal teaching and of mentoring preregistration nursing or midwifery students
