Job overview

Bollington, Disley, Poynton Team in East Cheshire NHS Trust is looking for an experienced community nurse who has the enthusiasm and positive approach to join a multi-disciplinary team with a vision to deliver the right care, at the right time, to help improve the lives of our local residents.

We actively promote personal development with a competency-based framework which will be provided to ensure support to successful individual.

The post holder will work within the Community Nursing teams of East Cheshire, in collaboration with colleagues in Therapies and Social Services, as part of  multi-professional working.

37.5 hours per week permanently & 28.50 hours per week fixed term for 12 months

Main duties of the job

To deliver physical, psychological and emotional care to patients in line with the patient care plan and under the direction and supervision of the registered case manager in the patient’s home or community-based facility. 

To report information to trained staff on progress of patients and any variance/change in patient/carers care management in a timely manner. To complete patient records in line with local record keeping standards and protocols 

To use IT systems for relevant tasks e.g. accessing patient records and results, completing record keeping.

To maintain a patient orientated approach towards care delivery, promoting a rehabilitative and self-care approach 

 To put person centred values into practice and to promote dignity in your day to day work with the individuals you support

To follow the Trust guidance in moving and handling and comply with the patient’s care plan. 

 To report any issues that may put health, safety and security at risk following the Trust Risk Assessment process. 

To report accidents/ incidents of patients or self on the Trust reporting system (datix) 

 To report any actual or potential abuse or concerns to the Team Lead and to summon help immediately in a crisis situation.

To report to manager/trained nurse any untoward incidences (e.g. complaints, accidents, defective equipment) and take appropriate action to ensure safety of patients, staff and relatives.

Qualifications

Essential criteria
  • Registered Nurse to degree/diploma level
  • Current registration with NMC (Nursing and Midwifery Council)
  • Evidence of recent professional development in an up-to-date portfolio

Knowledge

Essential criteria
  • Ability to assess clinical situations and act appropriately
  • Able to assess, plan and evaluate patient care, taking into account the needs and desires of the patients
  • Awareness of effects of ill health and the ageing process
  • Awareness of the principles and practice of SSKIN (Surface, Skin inspection, Keep moving, Incontinence, Nutrition).
  • Basic computer literacy including Knowledge of Microsoft office systems and use of email
Desirable criteria
  • Leg ulcer assessment and management, experience in palliative care/care of the dying
  • Understanding about NHS Long Term Plan agendas
  • Diabetes care / management

Skills and Experience

Essential criteria
  • Recent previous experience within a comparable role/transferable skills.
  • Demonstrable knowledge of assessment and therapeutic interventions in area of practice
  • Ability to prioritise and organise workload effectively
  • Experience of using electronic patient / service user record systems
  • Ability to communicate in a professional way with people at all levels of understanding either face to face, on the telephone, e-mail or written communication
  • Compassionate – with exceptional interpersonal skills with the ability to communicate effectively with staff, patients, carers and relatives (as appropriate) remaining sensitive and empathetic
Desirable criteria
  • Experience of working within a multi-disciplinary team.
  • Previous experience delivering care in a community setting
  • Experience in wound care assessment and management
