An exciting opportunity has arisen to work within Rapid Response/Stroke early supported discharge team. The role involves working with Community Support Workers, physiotherapists, Occupational Therapists & Nurses within the teams. The main objectives of the team are to expedite and facilitate safe and timely discharges from wards and emergency department areas, and provide resettlement, rehabilitation, care and medication support at home 

 You will need to be able to maintain a strong patient centred focus, whilst being adaptable, autonomous, relate well to colleagues across a range of disciplines in hospital and community, and able to work under pressure.

 The ideal candidate would have:

Discharge Planning skills Acute medical nursing experience Wound care skills

Experience of medication reviews

Clinical Experience of Stroke patients

Wound care skills

Knowledge and experience of community services

Valid car licence with use of care for business use 

You will be responsible for identifying and assessing patients located on wards and in the emergency department for suitability at home with Rapid Response/Stroke ESD  intervention. You will be expected to expedite discharges and provide assessment, nursing care, advice and information to patients, relatives and carers within the home or appropriate care setting. You will be aiding the Community Support Workers and Therapy staff to maximise patients’ independence and optimise their safety and quality of life, to maintain patients’ in their home, where possible. 

Secondment into the post may be possible for current Countess employees – please discuss this with your manager.
