Job overview
An exciting opportunity has arisen to join the established Heart Failure Specialist Team to assist with the running of the Heart Failure service.
This will include supporting the Heart Failure clinics to assist with face to face care for patients living with heart failure.
The role will also encompass supporting with the coordination of the Heart Failure service.
Main duties of the job
To work under the supervision of the Specialist Heart Failure practitioners as a knowledgeable and experienced member of the team, to actively participate in the assessment, planning, implementation, and evaluation of care by working collaboratively with the Cardiology nursing team and other Healthcare Professionals to meet the needs of patients and their families. The role includes the utilisation ,and updating of, the Trust’s Digital Clinical System (DCS) to include clinic appointment outcomes, supporting the management of patient appointments and managing patients visit to our Outpatients Department. An understanding of ICE is required, and the post holder must be comfortable with answering patients queries both face to face and when maintaining/monitoring the answerphone service. The role with comprise of mornings being spent supporting the heart failure team to deliver nurse led clinics, and the afternoon managing appointments, referrals and patients queries.

Detailed job description and main responsibilities
1. Taking and recording of observations of patient’s physiological state.
2. Monitoring and recording of blood glucose levels.
3. Monitoring and recording of ECG.
4. Carry out venepuncture.
5. Collecting blood products from the Hospital blood bank.
6. Involve patients, relatives/carers in the delivery of care, providing reassurance and support to assist their understanding and co-operation.
7. Contribute to the effective and efficient use of resources by ensuring that supplies are available, alerting senior staff when supplies are short, and by making suggestions to reduce waste and improve processes.
8. Prepare the clinical area for safe practice by ensuring that cleanliness is maintained, materials and equipment are available and ready for use, including clearing away afterwards.
9. Maintaining a healthy, safe and secure working environment.
10. Maintaining accurate documentation and reporting any concerns.
11. Contribute to the recording and storing of information, ensuring it is securely held, accurate, complete, and legible and can be retrieved promptly when required.
12. Provide escort and transfer for patients as directed by the Cardiology nursing team.
13. Recognise and respond appropriately to urgent and emergency situations.
14. Contribute to the development of services from the patient’s perspective, making suggestions for change and improvements, including taking part in patient and public involvement activities.
15. Ensure compliance with Trust policies, procedures and guidelines for self and others, by taking action/alerting senior management team if practice appears to contravene policy, or if concerned about any aspect of patient care.
16. Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, including full participation in KSF/appraisal, supervision, action learning and by maintaining a professional/personal portfolio of learning.
17. Comply with Infection Prevention and Control (IP&C) policies and procedures as appropriate to their role and responsibilities in their individual work setting. Staff are required to be personally accountable for their actions and be responsible for their own compliance in relation to IP&C polices, protocols or advice.
