Job overview

An exciting opportunity has arisen to work as a Band 6 Specialist Nurse for the Virtual Ward within the Central Cheshire Integrated Care Partnership Care Community Teams (CCICP). Ensuring the smooth and timely enrolment of patients deemed suitable to receive safe care and treatment in their own home.

You will be part of a dynamic team providing care and support to Virtual Ward patients with frailty, heart failure, and respiratory conditions. With a high proportion of complex patients, this role offers an excellent opportunity to enhance your clinical skills and knowledge. As the service continues to grow, additional specialties will be introduced, offering further opportunities for development and broadening your clinical experience.

You will  be joining a team upholding the trust values and behaviours, something the CCICP strongly feel should be embedded. You will ensure consistent delivery of the service and maintain communication and a joined up clinical approach across community and secondary care.  

The Virtual Ward service aims to promote early discharge from hospital and prevent unnecessary admissions by delivering and implementing treatment and care plans within patients’ own homes, ensuring safe, effective, and patient-centred care.

The Virtual Ward Service is a 7 day a week service (8am-8pm)

Full and part time hours available 

Please be aware that a UK driving licence is required for this post, as the role involves driving across the footprint of Mid Cheshire.

Main duties of the job

To manage a defined caseload of Virtual Ward patients, ensuring effective coordination, delivery, and evaluation of care. Utilise remote monitoring technologies to support clinical decision-making, enabling early identification of deterioration and appropriate escalation, while ensuring safe and effective care within patients’ own homes.

Undertaking holistic assessments and coordinate the delivery of individualised care as per the multi- disciplinary team to meet the patient’s physical, social, emotional, and environmental needs, encompassing a rehabilitative/ self-care approach where necessary. 

To work within the agreed framework for Community Nursing Services to provide holistic nursing care, advice and information to patients and carers, either by telephone, within the home environment or appropriate care setting in a safe and professional manner using evidence-based knowledge.

To provide skilled nursing care and ability to undertake clinical tasks including venepuncture, vital signs monitoring and wound care.

To maintain accurate personal and professional records adhering to MCHFT and Nursing and Midwifery Council policy and guidelines.

 Working effectively to support other team members in all aspects of the service.

To maintain all aspects of MCHFT Integrated Governance Strategy and requirements

Act at all times in a manner which illustrates respect for privacy dignity and confidentiality

At times be expected to support the wider community nursing team.

Mid Cheshire Hospitals NHS Foundation Trust (MCHFT) provides good quality, safe and effective healthcare to the people of Cheshire and beyond.

The Trust, which manages Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford, was established as an NHS Trust in April 1991 and became a Foundation Trust in April 2008.

Detailed job description and main responsibilities

The post holder will be responsible for the management and coordination of patients requiring the Virtual Ward service.

The Virtual Ward Specialist Nurse will oversee/undertake the ongoing assessment of patient needs, the development, implementation and evaluation of programmes of individualised care to patients as determined at Virtual Ward Multi- disciplinary Team meetings.

They will deputise as required in the absence of the team leader to ensure continued efficient and effective service delivery.

They will work as part of the integrated community health care team, working within and across professional and organisational boundaries covering the hours of service delivery. They will refer to and liaise with other services within the Central Cheshire Integrated Care Partnership, when patient need requires it. 

They will assist and support the Virtual Ward team leader in providing clinical leadership to the Virtual Ward team and to assist in leading the team in the provision of high quality, evidence based programmes of care.
