We are looking for an experienced Band 3 HCA to work within a busy community team. The post is based in Sandbach, working within SMASH care community.

The role includes caring for patients who are approaching the end of life. The post holder will undertake clinical tasks as delegated by the Community Nurse team and within sphere of competency.

You will be working without close supervision, and will therefore need to use your own initiative, whilst following the patient's care plan. You must also be passionate about making a difference to people’s lives, helping them to live as well as possible and to die with dignity.

As well as being able to work independently, you will possess good time management and organizational skills and be expected to be innovative and flexible to work in other areas - if required - to meet service need.

To join our dedicated, caring team, you will need excellent communication skills and emotional strength. We will expect you to have a talent for forging strong empathetic relationships with a variety of people, whilst providing palliative care. We will of course provide excellent on-going training and support.

Your role will include delivering a range of nursing interventions to patients in their own homes. This includes wound care, phlebotomy, medication administration and risk assessments. 

You will be adaptable, reliable and approachable, and will be expected to support your peers, the team leader and the organisation in promoting the Trusts Values and Behaviours.

Please note whilst your main base will be in Sandbach, you may be required to work across the footprint of Central Cheshire Integrated Care Partnership.

Travel is an essential part of this role; please demonstrate on your application form how you would meet this requirement.

Mid Cheshire Hospitals NHS Foundation Trust provides a full range of local hospital and community services for people across East Cheshire (population 399K) and West Cheshire and Chester boroughs (population 357K).

The Trust provides high quality planned and unplanned/emergency care, cardiac and critical care as well as child health, maternity services, and intermediate care across our sites at Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford. The Trust provides 500 beds and employs over 5,000 members of staff.

An extensive range of community services is provided across 26 medical centres and schools by our community arm, Central Cheshire Integrated Care Partnership, in collaboration with Cheshire and Wirral Partnership NHS FT and the South Cheshire and Vale Royal GP Alliance. With the formation of Integrated Care Systems in July 2022 the Trust became a strategic partner in both Cheshire East and Cheshire West and Chester ‘places’ and is part of the Cheshire and Merseyside partnership caring for a population of over 2.7million.

During the Coronavirus pandemic, the Trust was recognised nationally for its ‘Be Safe Be EquiPPEd’ campaign, which aimed to make the Trust as safe as possible for staff and patients, through supporting the correct use of PPE.

1. Function as an effective member of the multi-disciplinary team
2. Maintain clear concise and legible documentation adhering to prescribed standards and protocols
3. Use IT systems for relevant tasks e.g. accessing patient records & results, data collections and inputting data
4. Obtain consent in accordance with Trust Consent Policy
5. To report accidents/incidents on clinical incident reporting system of patients or self which occur whilst on
duty.
6. To maintain a patient orientated approach towards care delivery, promoting a rehabilitative and self-care
approach
7. Undertake phlebotomy duties as delegated and overseen by the registered nurse, working within the
policies and procedures of the Trust.
8. Undertake clinical tasks as delegated by the Community Nurse team and within sphere of competency.
9. Assist Community Nursing Team in management of patients with Long Term Conditions and in End of Life.
10. Recognise the need for referral and liaise with other health care and multi-agency workers as necessary
11. Participate in the induction and orientation programmes for all grades of staff who require community
experience as arranged by the Team Leader.
12. To learn and maintain an up to date knowledge of the blood tests performed and select the appropriate
sample tubes for the analysis requested and ensure that they are correctly labelled at the time of sampling.
13. Maintain the phlebotomy stocks, keeping them clean and in good order.
14. Follow Trust policy for any needle stick incidents and ensure reported accurately on Ulysses system.
To dispose of all syringes, needles, etc. in accordance with health and safety procedures
