Cardiac Rehabilitation Specialist Nurse, band 6,  37.5 hours per week available  WCH base 

To assist in the provision of a highly specialised cardiac rehabilitation programme of care, support, exercise and education. To encourage and motivate risk factor reduction and psychological  adaption for patients and their carers diagnosed with cardiac conditions pre and post revascularisation/ treatment.  To liaise, support and be an expert resource for the multi-disciplinary team from secondary, primary and tertiary care involved with the care and rehabilitation of cardiac patients

We are a small friendly team of cardiac rehab nurses, working within the West Cumberland Hospital  providing a highly specialised Cardiac Rehabilitation  programme of care, support, education and exercise working closely with the Physiotherapy Dept.

We offer opportunity for continuing professional development within the role.

Monday to Friday working.

Main duties of the job

To demonstrate expertise within cardiac rehabilitation field. Identify and assessing patients with a wide variety of cardiac symptoms, providing specialist advice and guidance to patients and other members of the MDT. This role provides education, counselling and reassurance to patients.

 To act as a role model for other Health care professions, acting as a patient advocate. To Provide along with the MDT an out patient exercise programme. To provide an initial assessment and clinical support for exercise.

Maintain accurate records, take part in the NACR audit.

Maintain and continue professional development within the role.

Comply with NMC and all relavent codes of conduct at all times.

The Trust is committed to safeguarding adults and children therefore if the post is subject to the Rehabilitation of Offenders Act (ROA) 1974 (Exceptions) Order 1975 (as amended) the successful candidate(s) will be subject to a criminal records check with the Disclosure and Barring Service (DBS). The cost of the DBS must be met by the successful candidate(s) through salary deduction. DBS charges are as follows if applicable to the post:

Standard Check: £18 + Administration cost of £3.85 = £21.85. Deducted at £6.85 from 1st month salary and £5 deducted from salary over following 3 months.

Enhanced Check: £38 + administration cost of £3.90 = £41.90. Deducted at £11.90 from 1st month salary and £10 deducted from salary over following 3 months.

Qualifications

Essential criteria
  • Registered Nurse
  • Relevant teaching qualification

  • BACPR principles and practical implication of exercise training for cardiac patients or will in to undertake BACPR Level 1 & 2
