Job overview

Derbyshire Community Health Service Trust (DCHS) has an exciting opportunity for a cardiac professional to join our cardiac rehabilitation team. 

The Cardiac Rehabilitation service covers North Derbyshire to deliver exercise and education to cardiac patients. 

We are looking for a Cardiac Nurse / Physiotherapist with understanding and experience in the delivery of rehabilitation, and the confidence to integrate into a innovative and passionate team. 

Main duties of the job

The main duties of the role include, but are not limited to, assessing patients safety to enrol onto the exercise and education programme. 

Assisting in the delivery of safe and prescriptive exercise, whilst monitoring, recognising and then adapting exercise when required. 

You will be expected to travel across North Derbyshire, aiding in functional testing and exercise delivery. 

Contribution to service development and innovation will be key, as we continue evolve to meet the complex needs of our patients.

Support colleagues in the delivery of exercise, adapting where necessary and progressing when needed. 

Functional testing in clinical assessments. 

Telephone support and assessment of patients. 

Inputting patient data onto system one, AMaT, and at times the National Audit database.  

Contribute to innovative ideas for better working ways and better patient care. 

Administrative roles and activity, including digitisation.

Essential criteria
  • Evidence of recent training to cardiac rehabilitation
  • NMC / HCPC Registration
Desirable criteria
  • Education to or working towards degree level training

Experience

Essential criteria
  • Experience in cardiology / cardiac rehabilitation
Desirable criteria
  • Experience in service development and transformational change process
  • Community care delivery or cardiac rehabilitation

Skills and attributes

Essential criteria
  • Excellent communication and interpersonal skills
  • Inquisitive and open minded
  • Good IT skills, including microsoft office, excel, TPP
