Job overview
Thrushel Short Stay Ward is a fast-paced acute medical unit at Derriford Hospital, providing assessment, treatment and short-term care for adult patients with a wide range of medical conditions. The ward focuses on rapid clinical assessment, early intervention and timely discharge, supporting patient flow through the hospital and improving patient experience.
The post holder will work as part of a multidisciplinary team delivering safe, effective and compassionate care in line with Trust values and clinical standards. The role requires excellent communication, organisational skills and the ability to work flexibly in a dynamic environment with high patient turnover. The post holder will contribute to patient assessment, care delivery, discharge planning and service improvement initiatives, ensuring high standards of care and a positive patient experience.
***Preference will be given to internal Trust staff, as well as ‘Priority’ and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***
Main duties of the job

Deliver high-quality, person-centred care to patients under the direction and supervision of a registered nurse.

Support assessment, planning, implementation and evaluation of individualised care plans.

Undertake and record routine clinical observations and measurements, escalating concerns appropriately.

Carry out delegated clinical procedures in line with training, competence and Trust policies.

Support patients with activities of daily living, promoting dignity, independence and wellbeing.
-
Accurately document care delivered and contribute to effective handover and communication within the multidisciplinary team.

Recognise and respond to changes in a patient’s condition, escalating promptly to the registered nurse or appropriate professional.

Support safe discharge planning by providing patient education and liaising with relevant services.
-
Maintain infection prevention and control standards and contribute to a safe clinical environment.

Participate in health promotion, patient education and support for carers where appropriate.

Contribute to service improvement, audit and quality initiatives within the clinical area.

Maintain professional accountability, continuing professional development and compliance with NMC standards and Trust policies.

Knowledge and experience
Essential criteria
- Registered Nurse Associate Training completed
Desirable criteria
- Demonstrable experience of acute hospital setting
- Completed Preceptorship
Qualifications

- Having gained Foundation Nursing Associate degree
