Specialist Practitioner in Cardio Pulmonary Exercise Testing

Job overview

To provide and manage a Consultant supervised cardio pulmonary exercise testing  (CPET) facility in the preadmission assessment clinic to risk stratify and optimise patient fitness before surgery

The post holder will be responsible for implementing, co-ordinating and delivering effective, high quality care for the pre-operative patient, following a client-centred model of practice and ensuring the area of work runs smoothly and efficiently

 

To make appropriate verbal and written assessment to gain information relating to a patients surgery

 

To act on such assessments in preparing and supporting the patient’s pathway, referring to outside specialist help if required

 

Can show interest/ experience in working as part of an MDT in research as an associate, PI or CI.

Provide high standards of patient care for those patients and their relatives attending the pre assessment department

Use of audit and data collection to demonstrate results of service and ensure these are shared with wider team

To lead by example demonstrating effective role modelling to all members of the team and demonstrate clinical expertise

 

***Preference will be given to internal Trust staff, as well as ‘Priority’ and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

 

Main duties of the job

Suitable post for an experienced registered practitioner who wishes to contribute to the work of the Pre-Admission Assessment Clinic through the provision of high-quality exercise testing and pre-assessment services  

To contribute to the overall efficiency and development of the pre assessment service.

To demonstrate clinical expertise in the pre-assessment unit

Ensure adherence to the pre-operative assessment policy.

Working with the leads of the pre-assessment service to identify and develop clinical changes to working practice and working in different ways to enhance the service provision and patient experience

To provide daily clinical leadership and management and support for the service.

Detailed job description and main responsibilities

CLINICAL PRACTICE

  • Ability to run and co-ordinate the CPET service on a daily basis and acting  on such assessments in preparing and supporting the patient’s pathway, referring to outside specialist help if required and escalating concerns as necessary.
  •  Actively monitor and make preliminary assessment of exercise testing.
  • Applies clinical judgement to identify appropriate clinical and physical interventions which may include a range of options.
  • Able to translate theory into the practical delivery of care for patients within the pre-assessment service
  • Recognise and act upon appropriately situations that may be detrimental to the health and wellbeing of the individual
  • Will be expected to take and interpret clinical information and apply this to ensure individual patient needs are met.
  • Ability to complete an accurate and comprehensive pre-assessment of patients requiring surgery by undertaking a targeted history ensuring they are fully prepared physically, psychologically, emotionally and socially for their procedure
  • Advise patients on the promotion of health and prevention of illness
  • Collates and analyses information on patient treatment plans/timescales, identifying bottlenecks and developing action plans to be implemented with the support of the multi-disciplinary team
  • Works with the multi-disciplinary team to support the organisation and monitoring of the patient journey in order to provide continuity of patient care.

Person specification

Knowledge and Experience

Essential criteria
  • Excellent interpersonal and communication skills written & verbal.
  • Effective organisational and management ability with proven ability to work without Supervision.
  • Able to work effectively as a member of the Multi-disciplinary team and independently
  • Evidence of ongoing continuous professional development
  • In addition to the essential requirements of the role, the postholder is required to maintain and improve professional knowledge, skills and competence in particular relation to exercise physiology and patient communication; ensure the delivery of evidence-based practice in line with the Professional Code of Conduct
  • Competency in ECG analysis or willingness to learn.
  • Significant knowledge in Good Clinical Practice and Research Framework set out by the NIHR
  • Demonstrable experience in: Venepuncture, Observation recording, Arterial Blood Gas and BM analysis.
Desirable criteria
  • Demonstrable experience in CPET testing
  • Existing knowledge of pre assessment service and CPET testing
  • Demonstrable experience of pre – operative assessment
  • Previous demonstrable experience at band 6.
  • POETTS Accredited / In process of attaining.
  • Proven research publications or involvements, specifically relating to CPET and/or physiological sciences
  • Knowledge of Budgetary Control
  • Demonstrable experience as a PI/CI for a research case.
  • Demonstrable experience and execution of appropriate risk assessment and management

Qualifications

Essential criteria
  • Registered Allied Healthcare Professional (incl ODP), Registered Nurses. Registered Healthcare Scientist/Registered practitioner with Academy of Healthcare Sciences and/or RCCP.
  • ILS
  • Evidence of continuous professional development (CPD)
  • Recognised Leadership Qualification (e.g ILM)
  • Computer Literate
Desirable criteria
  • BSc in a Healthcare related Degree or equivalent
  • MSc or proven specialist knowledge in CPET
  • Mentorship or teaching qualification.
  • Arterial Blood Gas (ABG) extraction and analysis.

Aptitude and Abilities

Essential criteria
  • Dynamically monitor, identify and respond appropriately to patient deterioration. Initiate basic life support/cardiopulmonary interventions if/when required and have adequate competence in emergency handover.
  • Assess patient’s cooperation with test requirements and appropriate, scientific analysis of test results to aid shared decision making in anaesthetic review
  • Methodical & Organised
  • Assist with the development of the specialist practitioner service.
  • Proven ability to teach and support staff with their learning of ECG’S and other clinical investigations (Spirometry, Arterial Blood Gas analysis)
Desirable criteria
  • Participates in the education of patients and carers.
