Pan Dorset Learning Disabilities Intensive Support Team

Part Time 18.75 hours Band 6 Specialist Practitioner Out of area monitoring. 

This role will involve travel to out of area hospitals it may involve over night stay dependant on distance to the hospital. 

You must be a highly skilled Learning disabilities practitioner with knowledge and skills of working with people with duel diagnoses of learning disabilities, mental health, behaviours that challenge, placement break down and Forensic history.

Experience of lone working and working with in a multidisciplinary team.   

Main duties of the job

Main Duties are to provide Good quality Out of area Monitoring for People with a Learning Disability placed in Out of area specialist Hospitals, 

The purpose of the role is to be the local Contact and facilitate face to Face monitoring visit at a minimum of 3 monthly.

To support People to identify Goals for discharge.

Support development of be spoke person centred discharge plan. Along side multidisciplinary team both in patient and locally. 

Monitor clinical treatment pathway whilst in specialist out of area hospital.

Report out come of monitoring to local commissioners, Community Learning Disabilities Team, the provider of service, individuals and family and carers.
