Job overview

We have a developmental role for a 14 month secondment in The Vale District Nursing Team based in Shaftesbury Hospital. Community experience is essential.  

The team has a breadth of experience and a shared goal to offer the highest standard of care to our patients. We work closely with other health and social care professionals across primary and secondary care, as well as with patients, families and carers in their own home.

Your role will be to lead the team and case manage the provision of care within an individual's home or residential home, including palliative care and long term conditions management. You will work autonomously using your own initiative to deliver holistic care to adults with physical health needs. 

You should possess excellent leadership abilities, interpersonal skills and time management skills. You should have experience in caseload management and be able to prioritise work. The role is diverse, challenging and extremely rewarding. The role is part time and will include working  weekends and bank holidays on a rota basis .

Main duties of the job

To provide a high quality person centred approach to care delivery which always considers people’s safety, privacy and dignity.

To provide specialist care and interventions, assessments, treatments and managing patients on a caseload, as part of the wider multi-disciplinary care team.

To provide specialist advice to others regarding the management and care of patients / service users.

To devise specialist programmes of care for other professionals to deliver and ensure they are delivered appropriately.

To plan, implement and review health improvement programmes in a range of settings.

Provide supervision, mentorship, training and clinical support to students and trained practitioners.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.  Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check

Person specification

KNOWLEDGE, SKILLS AND TRAINING

Essential criteria
  • Registered practitioner to degree/diploma level supplemented by post registration diploma level specialist training and/or short courses or demonstrable extensive experience in the relevant specialty
  • Membership of the relevant Professional Body
  • Practice Assessors' course/ experience

JOB SPECIFIC EXPERIENCE

Essential criteria
  • Experience at Practitioner Band 5 level
  • Experience of specialist working
  • Experience of developing specialist programmes of care for an individual or groups of patients/clients and of providing highly specialist advice

MANAGERIAL/SUPERVISORY EXPERIENCE

Essential criteria
  • Experience of providing clinical supervision and mentoring to junior staff
  • Experience of devising and delivering training

INFORMATION TECHNOLOGY/RESOURCES

Essential criteria
  • Experience of using electronic patient / service user record systems
  • Able to analyse data and produce reports using Microsoft Excel and Word
