The role of the SPA Clinical Triage Nurse is to receive referrals from a diverse range of people & services, triage the referrals & ensure that they reach the right service in the shortest time.  The team also provide significant support to the Virtual Frailty Ward at their daily MDT meetings.  SPA Triage monitor the Health Call system which is utilised by heart failure patients, ensuring that out of range observations are escalated & acted upon.  Palcall is a further element of the SPA Triage role; this is a telephone support & signposting service for people deemed to be within their last 12 months of life.
