Main duties of the job

We are seeking a compassionate and skilled Band 6 Community HIV Specialist Nurse to deliver high‑quality, person‑centred care to adults living with HIV across Brighton and Hove.

You will manage a specialist community caseload, undertaking holistic assessments, planning and evaluating care, and supporting patients to manage their condition and live well at home. The role involves working autonomously while collaborating closely with primary care, acute HIV services, voluntary organisations and the wider MDT to ensure seamless, coordinated care, particularly around hospital discharge.

You will provide expert advice on HIV, sexual health and treatment adherence, act as a clinical role model, and contribute to service improvement and audit. You will also support and line manage Community HIV Support Practitioners, ensuring safe delegation and high standards of care. Strong communication skills, clinical judgement, and a commitment to reducing stigma are essential.

Detailed job description and main responsibilities

The post holder will be responsible for the delivery of specialist HIV nursing care to adults in community settings, managing a defined caseload of patients with complex physical, psychological and social needs. This will include undertaking comprehensive assessments, developing, implementing and evaluating individualised care plans, and promoting self‑management and treatment adherence.

The post holder will work closely with acute HIV services, primary health care teams, hospices, continuing care services and statutory and voluntary organisations to coordinate care, facilitate hospital discharge, and ensure continuity of support. Joint visits, multidisciplinary meetings, case conferences and discharge planning meetings will be undertaken as required.

The role includes providing expert advice and education to patients, carers, community staff and partner agencies on HIV, sexual health, long‑term condition management and psychosocial issues, including stigma and discrimination. The post holder will act as a specialist resource and clinical role model, promoting evidence‑based practice across the service.

The post holder will provide line management, supervision and support to Community HIV Support Practitioners, ensuring safe delegation of care, staff development, appraisal and performance management in line with Trust policy.

They will lead and contribute to clinical governance activities, including audit, risk assessment, incident reporting and service improvement initiatives, ensuring compliance with Trust policies, NMC standards and CQC requirements. Accurate and timely documentation will be maintained, and activity recorded to support service monitoring and development.

The post holder will actively participate in education, training, audit and research activities and contribute to the ongoing development of the Community HIV Specialist Service.

Qualifications

  • RN first level nurse (Adults)

  • Post graduate level degree or equivalent
  • Mentoring Course or equivalent assessor

Experience

Essential criteria
  • HIV clinical experience and case management experience.
  • Evidence of ability to maintain and monitor high standards of care.
  • Evidence of continuing professional development
  • Experience in supervising or line managing junior clinical staff
Desirable criteria
  • Up-to-date knowledge and skills, particularly in Long term Condition management and the care of patients with HIV
  • Experience in Research and clinical audit
