Community Liaison Nurse - Admission Prevention Team
An exciting opportunity has arisen within the Admission Prevention Team based at the Royal Sussex County Hospital for an experienced nurse. The aim of the team is to triage and assess patients that come into the emergency department by ambulance, with a view to preventing admission to hospital.
Main duties of the job
The post holder will work within the A&E department based at the Royal Sussex County Hospital. The post holder will act as an ambassador for Sussex Community NHS Foundation Trust and play a key role in supporting at the front door of A&E in preventing hospital admissions.
Utilising their knowledge of the community services and clinical skills the post holder will work closely with the A&E staff to ensure seamless, safe and effective discharge plans are in place for the person to return back to their usual place of residence as soon as possible, preventing admission to hospital.
The post holder will play a key role in using data from incidents, feedback and observation to identify potential improvements or changes within the service which will support effective safe discharge

Detailed job description and main responsibilities
The post holder will have excellent clinical decision-making skills that will support the A&E staff in the management of discharging patients into the community and community beds.
The post holder will support with the induction of new staff.
The post holder will attend MDT meetings, and support A&E staff with complex discharge functions as required.
To provide training and education to A&E staff around what support community services can provide and support in discharge management & processes.
To ensure records and data are maintained in a timely manner and in line with information governance and data sets required for the service.
To proactively identify patients who are already known to community teams and ensure that discharge back to the community is planned as soon as clinically safe to do so

Qualifications
Essential criteria
- Registered Nurse, with current NMC registration
- Post registration study or qualification
Desirable criteria
- Mentorship qualification
- Leadership qualification
Experience
Essential criteria
- Minimum 3 years’ experience in Band 5 role
- Experience of working within a community or hospital setting
- Excellent interpersonal skills
- Experience of working with multi-disciplinary teams
Desirable criteria
- Experience of providing supervision/appraisal
Skills and Knowledge
Essential criteria
- Evidence of a range of advanced clinical assessment skills appropriate for the role
- Advanced communication skills including difficult conversations, negotiation, managing conflict
- Promotes patient centred care including self-management
- Demonstrates application of evidence-based practice
- Knowledge of best practice discharge planning processes
- IT literate
- Understanding and knowledge of current issues in the NHS
- Current knowledge of community services
- Demonstrates competency in a range of clinical skills, reviewing and interpretation of patient records
- Ability to work autonomously and make own decisions competently, confidently and efficiently

- Confident in dealing with both clinicians and senior managers
- Demonstrates a high level of understanding of self and able to identify personal limitations and show openness to address them
- Calm and rational approach to situations where conflict is likely to happen

- Ability to travel between sites; car owner/driver
